The Region 7 IDN full implementation plan has been submitted!!  The state will do a general review and provide feedback. We would like to complete that process before sharing it with all the Region 7 partners. Stay Tuned! Following that we will learn our scoring in November.

NCHC will work to facilitate opportunities for partners to come together in the creation of their proposals, please notify April if your are interested.
Full proposals are due September 6, 2017- must include an evaluation and a work-plan, use the template of your choice.

September 21st- Fall Region 7 Full partner quarterly meeting with extended agenda to provide time to debut all the program deliverables in the implementation plan. Lunch will be provided.
