1095-C forms are available!
The Affordable Care Act requires large employers to provide IRS Form 1095-C to all employees by March 2, 2018. Form 1095-C provides information about the health insurance offered to you by the School District in 2017. Forms are now available electronically.

Why should you consider giving consent to receive the 1095-C form electronically?

Agreeing to receive this form electronically means you will receive the form as soon as it becomes available! The PeopleSoft system is secure and password protected.

Also, the District will save paper and postage costs.

Without consent, the form must be mailed.

Electronic consent may be withdrawn, should you change your mind in the future.

It is simple to provide consent to receive you 1095-C electronically in the future. Follow the steps outlined in the link below.
