  40 Square Dental and Vision Update!
You are receiving this email because you have either enrolled in the dental, vision, or both plans through 40 Square. We wanted to provide you an update on one of the hot questions we are receiving right now from you, our 40 Square plan participants.
 
I don’t have our dental or vision ID cards and we have appointments scheduled. What should we do?
 
Currently, Reliance Standard is working hard to get all new January 2018 business set-up, eligibility loaded, and ID cards out the door. Unfortunately, at this time, they expect it to be two to three weeks before ID cards are sent out. Coverage is in place as of 1/1/2018 for those who elected coverage for dental and/or vision, however, it is an administrative delay that is holding up ID cards and eligibility being loaded into the system. If you have upcoming appointments there are a couple of things you can do:
 
1) Let your provider know that you do have coverage as of 1/1/2018, but you are still waiting on ID cards. Most providers understand that this time of year.

2) You can send an email to info@40Square.coop with the appointment details, date, provider name and phone number and who is scheduled for an appointment. We can request that Reliance or VSP reach out to the provider and let them know the current administration delay.
 
Regardless of which of the above you choose to do, once you get your ID cards in the mail we would encourage you to contact your dental or vision provider to provide anything additional on the ID card they may need to have claims processed correctly on your behalf.
