A Second Opinion Provided For A Patient 
In Rural Mexico

More and more, baby-boomers, digital nomads, and others are choosing to settle in Central and South America and parts of Asia. 

In turn, RCM is meeting an increasing number of calls from around the world for our consulting services and second opinions.  

A case in point, we recently completed a case for a patient living deep within rural Mexico.
Case Study:                                     
Jim is a baby-boomer and self-described hippie. Originally from Vancouver, he married, had a daughter named Rainbow, and worked as a truck driver. He liked the freedom of the road.
 
When he decided to retire, he and his wife Evelyn figured they could live well on a small income in rural Mexico. Rent for an apartment was going at about $150 per month. They made the move and cancelled their BC Medicare.
 
One year later Jim developed chronic lymphocytic leukemia (CLL). He became symptomatic with weight loss, night sweats and pneumonia. The village doctor sent him to a local hematologist in a somewhat larger community, not too far away. The hematologist proposed a protocol called BR, which is Bendamustine plus Rituxin. This cost Jim about $1,000 per month for six months.
Rupert Case Management Is Retained                                    
Both Jim and Evelyn had questions about the treatment. They considered returning to B.C. for care, but Jim could not afford to wait the requisite three months to regain provincial healthcare coverage. He needed treatment sooner.
 
Jim's family in Canada retained RCM to provide a second opinion.  We retained a Spanish-speaking oncologist at Duke University to review Jim's clinical records, which were in Spanish, and the suggested BR protocol.
 
A leading expert in his field, the Duke specialist made recommendations about dosing and pre-medications. He agreed with the decision of the local hematologist. We provided Jim and Evelyn with a transcript of our discussion with the specialist at Duke, and ensured their complete understanding.
Favourable Outcome                                     
Jim, Evelyn, and their family are now reassured that Jim is getting the right treatment. They are also greatly relieved to know he can contact the oncologist at Duke with any further questions or concerns, through his team at RCM.
