In celebration of National Advance Care Planning Day on April 16th this months featured topic is Advanced Care Planning and Health Care Consent in Ontario

We have recently launched  Southeastern Ontario Portal for Advance Care Planning [ACP] & Health Care Consent [HCC] to share Ontario relevant information with health care providers and people living in Ontario.

If you haven't already taken our free online course for health care providers we invite you to start today by previewing Lesson 1 on sageducation.ca. Please note that you will need Adobe Flash Player to run the course.
 
We would like to congratulate the Canadian Frailty Network on receiving  $23.9 million in renewal funding from the Government of Canada's Networks of Centres of Excellence program.To learn more about frailty see our listing of resources on sagelink.ca.

Age-Friendly Communities

The Age-friendly Kingston Community Alliance (AFKCA) is looking for individual seniors (65 +) to join the Alliance. Preference will be given to seniors who live in Countryside District.The Alliance meets during the day, once per month for two hours at a central Kingston location. Background information on the Alliance and the application form can be found on the Seniors Association website. Deadline for applications is May 31, 2017

Speak Up Ontario has developed a 2017 National ACP Day campaign to remind individuals to think about the people in their community and suggest ways to have conversations about their health care. Click here to see their campaign resources.

In March, Senior Friendly Hospitals highlighted SFH ACTION (Accelerating Change Together in Ontario) - a collaborative of 87 hospitals across Ontario that have been engaged in process improvement initiatives which promote senior friendly care. To learn more click here.

Part to the challenge in initiating Advance Care Planning into our practice is understanding the differences in legislation here and across the country and the world.
