CHI was named a finalist in the Innovation category in the 6th Annual Long Island Imagine Awards for its ground-breaking Transitional-to-Permanent Housing Program (TPH).
The Long Island Imagine Awards was created to offer formal acknowledgement to some of Long Island’s most effective and innovative nonprofit organizations.
Winners will be announced live at the awards gala at the Crest Hollow Country Club on Tuesday, May 1, 2018.
Transitional-to-Permanent Housing (TPH) is a twelve-month program that answers a crying need for homeless families and individuals who need more than just an apartment to succeed. Emergency housing has one over-arching goal, which is to move the homeless into permanent housing as quickly as possible. However, many homeless people have barriers to keeping that housing, including mental illness and lack of life skills such as budgeting, that often result in a return to homelessness. The CHI TPH program serves as a “stepping stone” to permanent housing. It allows families and individuals to live in a CHI owned or leased home for up to one-year while they work with CHI case managers, meeting weekly to set and attain goals, including maintaining mental and physical health, staying substance free, obtaining employment and/or returning to school, with the ultimate goal of succeeding in permanent housing.
