Coyote Referral Form

***Back to School-Orientation***

We hope to see you tomorrow July 29th at our Back to School Open House/Orientation from 9:00am-12:00pm. Kinder & 1st Grade families are welcome to attend from 9:00am-10:30am, and 2nd thru 4th grades from 10:30am-12:00pm.  Students will have the opportunity to meet their teachers, bring in their school supplies, pick up their Welcome Packet which will include the family Zone Cards, purchase agendas and DH Spirit Shirts.  This is a great opportunity for our parents to learn about our Nutrition One lunch ordering process, as well as policies regarding Attendance and our C.U.T.S. program. If you are unable to attend please be sure to make arrangements with the office to pick up your welcome packet so that you have your Zone cards for dismissal on the 1st day of school.

Transportation Reservations

If your student will be utilizing our transportation services this year it is extremely important that you submit your transportation contract as soon as possible to ensure a space for them.  As a reminder, we have added 2 bus stops to further serve our Desert Heights Community, 91st Avenue & Union Hills, and 43rd Avenue and Peoria.  If you have questions, please feel free to contact Debbie Hamza @ (602)896-2900 or email at [email protected].

2015-2016 School Lunches

Please be sure to complete the registration process and order your   student's lunches prior to 10pm July 29th in order for them to receive lunches the week of August 3rd. We are happy to announce that Nutrition One will continue to provide lunch services for the 2015-2016 school year and the lunch cost will be $2.90 per student, per day. Additional a la carte items are available so please review the menu for details.

Returning Families: To ensure your student receives lunch when ordered, please access your student's account to update their grade level so it reflects the appropriate grade for the 2015-2016 school year.

New Families: We encourage you to sign your children up on the Nutrition One website. When creating a new account, you will be prompted to enter your child's teacher. Please note that this field does not need to contain a teacher's name. Instead simply put a "." in the space provided and it will allow you to continue with the registration process.

All Families:   The free/reduced online application can now be accessed on the school's website under the Parent Tab, Food & Nutrition Services menu.
  • Please have your student's ID number available before you get started. This can be found on the student's schedule, a previous student ID card or report card.  If you are a new family, and do not have this information, please contact the front office and we will be happy to pass along the information so you can get started.
  • Applications may take up to 10 days to process so please plan accordingly.
  • Your benefits will temporarily carry over from the prior year, but you MUST register and send an email to Maritza Delgado at [email protected] so she can update your temporary benefit.
If you have received an email from Miss Delgado regarding your notice of direct certification, please DO NOT fill out an application, your benefit has been confirmed. You must now begin the Nutrition One registration process.

From the Health Center

Chronic Illness Forms (CIF): A Chronic Illness Form and a Written Treatment Plan will be required if your child has Asthma or any other chronic medical condition.  This enables us to assist our students appropriately as well as identify any necessary medically excused absences.  For our returning students; a new CIF and WMTP is required every school year.  Please make sure to bring in any necessary medication(s) your child might need during the school year.

Medication Authorization Form:  A Medication Authorization Form must be completed with any medication that your child will need to take whether on a daily basis, temporary, as needed or in case of a medical emergency /anaphylaxis.

Allergy Verification Form:  A new Allergy Verification Form must be completed every school year.  For our returning students; once again, we need current and updated information to make sure your student's needs are met.   Please remember to plan your child's snacks and lunches accordingly, and make sure to bring in any necessary medication(s) your child might need during the school year.

You may obtain any of these forms from our web page or from our office.

ParentVUE

We are excited to announce that this summer we began migrating our Student Information System (SIS) from Schoolmaster to Edupoint Synergy. We decided to change systems for a variety of reasons, but attempting to address concerns from last year's parent surveys regarding the multiple system logins was definitely high on the priority list.  As we continue to progress through the setup and training that comes along with a system migration, we hope to have the new parent portal configured by mid-August.  The new system will be referred to as ParentVUE and will replace both FamilyLink and Edline.  With a single login, families will be able to access grades and classroom resources.  Additionally, ParentVUE offers our families some exciting features that weren't available in our previous systems including an application for mobile devices and tablets. We look forward to enhancing our school-home communication through the use of ParentVUE.  Keep an eye on the Tuesday Folder regarding the availability of this application in mid-August.
