Message from the Executive Director
Dear Community Supporters,
I hope that everyone is enjoying the sun and warm weather?
In between vacations this summer, staff are hard at work starting up new programs that will address health and wellness. All to often, Deaf and Hard of Hearing community members do not have equal access to health programs and services. As a result, health conditions may develop or worsen.
Deaf Access Services will soon offer health assessments and services to connect people with Medicaid, primary care and other necessary health services. Deaf Access Services will be providing these health home care management services as a partner of Health Homes of Upstate New York.
Health Home services
are not the same as "home health services". We won't be sending aides to your house! What we will do is make sure you have access to your doctor, and that you can easily order and pick up your prescriptions. We will also be able to connect you with specialists and help you understand your doctors discharge summaries and prescriptions. These are some examples of our care coordination services that will start in August/ September.
