With this issue of the newsletter we welcome Dr. Benjamin Alter, N.D., to our practice. Dr. Benjamin brings a calm, healing presence to Pacific Naturopathic and is a perfect fit both energetically and medically.  Read about some of Dr. Benjamin's experiences in the next article. Dr. Benjamin will be primarily working with Dr. Marcel in the cancer center and will also be seeing patients on his own.

This month, we also say a warm and sad farewell, for now, to Elijah Free, who has been our associate for 17 years. Elijah has been a multi-talented presence in the clinic. He is a brilliant herbalist, healer and bodyworker and has been a true and loyal friend to all of us for all these years.  He is moving on to practice alone for now, and to explore other opportunities for growth and service, including marketing opportunities for his herb company.  We will sorely miss him. After falling from his skateboard and landing in the emergency room at age 12, Dr. Benjamin knew he wanted to be a doctor. Despite the pain, he thought the physical mechanics of healing was amazing. 10 years later, another broken wrist led to further healing and transformation. After flying over the handlebars of a mountain bike, his right hand was immobilized for 2 months. His right-hand dominant, left-brained, scientific and analytical self was suddenly struck with creative inspiration. Still on course to be a doctor, his path took a drastic turn.

Upon completion of his degree in human physiology from The University of Arizona, Dr. Benjamin moved abroad to Hungary to work on a research project investigating brain cancer genomics. While away, frequent illness led to great self-awareness. He realized it was not any source of food, water, or pathogen that was leading to frequent acute symptoms. A stroke of insight led to a discovery that something deeper was missing - a sort of homesickness. He began a daily meditation and centering practice. Through taking dominion over thoughts and inner experiences he was able to shift outer experiences. Amazed by this experience of the mind-body connection, Dr. Benjamin's course of becoming a healer was forever changed.

Upon returning to the US, Dr. Benjamin began pursuing a Master's of Arts in Spiritual Psychology at the University of Santa Monica. Through deep inner work of identifying, owning, loving and forgiving all aspects and experiences, a new definition for healing was formed. This ever-present transformation continues to mold his day-to-day life, while also intimately informing his approach to medicine. Since entering into the field of Naturopathic Medicine, Dr. Benjamin has been devoted to the preservation of traditional healing modalities that call upon the body's innate self-healing ability. He believes that each individual is truly their own healer with his role being supportive as a teacher, also offering the most gentle and effective medicines as possible. 

When not with patients or keeping up to speed with the latest health trends, you can find Dr. Benjamin staying active in any number of ways. Whether cycling, doing yoga, hiking, or jumping around in his daily high intensity interval training, he loves to nourish his most with mindful movement. Dr. Benjamin works to balance his energetic ways with more restorative activities such as writing and his daily meditation practice. We mustn't overlook his most profound joy, spending time with his beloved partner Susanna, an emerging Naturopathic Doctor herself. They enjoy getting dirty in any organic garden, preparing plant-based cuisine, and doing anything outdoors. They believe their relationship itself also presents the grounds for immense healing, growth, and loving.

Musings on Digestive Issues

As with everything in medicine, understanding of bowel dysfunction and related digestive issues has evolved over the years. Prevalent conditions and diagnoses come and go. Whereas in the past, parasitic and bacterial infections were most predominant, now allergic, autoimmune and inflammatory conditions are increasingly common. 
 
Small Intestinal Bacterial Overgrowth (SIBO) is now thought to be a predominant cause of what has been known as Irritable Bowel Syndrome (which was simply a renaming of the condition of alternating constipation and diarrhea).  Maldigestion which responds to the use of supplemental enzymes is now advertised as Pancreatic Exocrine Insufficiency. (Radio announcements question and advise. "Do you have gas and bloating? You may have "PEI". Check with your doctor now for prescriptive help!") What was once heartburn became Gastro Esophageal Reflux Disease.
 
There is an increasing understanding of the connection between gut health and autoimmunity, gut health and obesity, gut health and mental health.  Leaky gut (gut permeability caused by infectious disease, use of antibiotics, hormonal change and/or stress) allows larger molecules to penetrate the gut wall, generating an immune response which may evolve into autoimmunity, as with gluten sensitivity and Hashimotos Disease (autoimmune thyroiditis). Obesity is promoted by the absence of specific probiotic bacteria which should be present in the gut micro biome.  There are more receptor sites in the gut for the calming neurotransmitter serotonin than there are in the brain.
 
As science uncovers multiple underlying causes of digestive discomfort, old diagnostics are revisited and new diagnostic testing becomes available.

In naturopathy, we have long relied on variations in the Comprehensive Stool Analysis Profile or "CSAP". These tests allow us to sort out micro biome, pancreatic enzyme, immune response  and inflammatory issues. (Are parasites present in the sample and are they associated with the digestive disturbance about which the patient is complaining? Is there a lack of beneficial bacteria?  If a microbial issue, to what is the overgrowth of yeast or bacteria sensitive? Is there appropriate response to antigens by the gut wall? And so on.)
 
Hydrogen and methane breath tests, long the standard for SIBO testing, are called into question, as newer blood tests are under consideration. The Anti-Vinculin and anti-CdtB blood tests evaluate the role of earlier food poisoning in both SIBO and IBS.

Elastase tests (available on the CSAP)  evaluate pancreatic function.  Labs such as Cyrex  evaluate some 20 variables of gluten sensitivity, as well as offering an intestinal antigenic permeability screen detailing autoimmune reactivity relevant to gut function.
 
Suggested treatments evolve around the new understandings and diagnoses.  What with all of this, it is well to take a step back and remember that, on another level, gut issues are in the energetic domain of the third chakra, and speak of what we take in and retain or eliminate on all levels, whether physical, mental, emotional or spiritual. We're talking here of negative thought patterns, dysfunctional behaviors, dietary issues, inappropriate supplementation or medication. These must all be addressed, whether with dietary and lifestyle change, and/or with energy medicines and mudra, mantra and yogic asana.

Understanding Digestion

 
The two most common complaints we see in our office are fatigue (lack of energy) and digestive disturbances.  We've talked about fatigue in several newsletters (please click HERE  for our newsletter archive)
 
Let's start with the digestion basics.  Digestion has two distinct parts: mechanical and chemical.  Mechanical digestion takes place in the mouth as soon as you take the first bite of a meal. Chewing breaks the food into pieces that facilitate the chemical part of the process.  Saliva begins the  process  of chemical breakdown of food down into a form your body can absorb and use.
 
As food  passes through the GI tract, it mixes with digestive juices, causing large molecules to break down into smaller molecules. The  body  then absorbs these smaller molecules through the walls of the small intestine into the bloodstream, which delivers them to the rest of the  body .  After the nutrients have been extracted via this two-fold digestive process, the processed waste is eliminated.
 
Simple enough, yes?  Then why do many of us have symptoms like acid reflux, heartburn, nausea, vomiting, diarrhea, constipation, gallstones, belching, burping, flatulence, abdominal pain and hemorrhoids?  Why do we have diseases like gastric and duodenal ulcers, GERD, celiac disease, diverticulitis, irritable bowel syndrome, ulcerative colitis and Crohn's? There are as many answers to these questions as there are people who have these symptoms and diseases.  Fortunately, we have tests and tools that we can use.
 
Dr. Connie mentioned the Comprehensive Stool Analysis Profile (CSAP) and other tests we use to evaluate digestive function and biochemistry in her article, below.  I'd like to offer some practical information on maintaining digestive health.
 
Choose whole, unprocessed foods.  When making food choices, the body understands "simpler" and gets confused with complex, "gourmet" combinations. 

Discover and eliminate food allergies. We can help you find both immunological and functional food allergies and sensitivities through both testing and elimination/challenge.

Treat any infections or overgrowth of "bad" bugs.  The CSAP test can shed light on your own specific biome, including friendly bacteria, pathogenic bacteria and yeast, and offer treatment suggestions.

Replenish your digestive enzymes. The fact is that as we age, our capacity to produce digestive enzymes diminishes.

Rebuild your inner forest of friendly bacteria. Probiotics can be soothing and restorative.  But, with so many alternatives on the market, which specific flora do YOU need?  The CSAP test can answer this question.

Heal your gut lining.  This is a complex multifactorial process that takes time and focus.  We can help you determine which approach is best for you.

Know your digestive triggers.  The triggers for digestive upsets vary from person to person. An easy way to learn which foods upset your digestion is to keep a food journal.  Include in the journal the foods and beverages you eat and medications and supplements you take. Note your moods, sleep quality and digestive function. Do certain events and situations seem to trigger your digestive problems? Does travel make you unravel? Do menstrual hormones wreak havoc with your digestion? If you suspect a connection, plan so you can anticipate, avoid, or at least prepare for such tricky situations.

Manage stress for a peaceful inner world.  Does stress seem to cause or aggravate your digestive problems? (Having frequent digestive flare-ups can be, in itself, a stress factor.) You may be able to relieve stress with relaxing pursuits such as exercise, meditation, yoga and fun hobbies.  

Finally, beware of miracle cures for digestive problems.  Any medical condition that presents treatment challenges will generate an entire industry capitalizing on a person's anxiety about their health. Many of these miraculous cures are expensive, ineffective, or downright dangerous. Consult with your favorite health practitioner before trying any alternative therapy or supplement.

Calming Hearburn

 

The conventional understanding is that heartburn and Gastro Esophageal Reflux Disease (GERD) are issues that can be resolved by decreasing stomach acid. This is accomplished with 3 classes of medication: antacids, H2 blockers and Proton Pump Inhibitors (PPIs).
 
Antacids neutralize stomach acid. These are common over the counter tablets such as Tums and Rolaids, or Maalox, Mylanta and Gaviscon. Baking soda works too. Antacids are relatively benign, though they can cause variable levels of acid, decreasing the bioavailability of certain common drugs such as tetracycline.
 
H2 blockers block the action of histamine on the parietal cells on the gastric wall. The parietal cells would otherwise produce acid. H2 receptor agonists include Tagamet (cimetidine) Zantac (ranitidine), and Pepcid (famotidine). A concern, particularly with Tagamet, is that these drugs can inhibit enzymes which control the metabolism and excretion of other drugs such as Warfarin and calcium channel blockers, thus increasing serum levels of these drugs.
 
Proton pump inhibitors, as the name implies, shut down proton pumps which pump acid into the stomach. More general than H2 blockers, these drugs suppress histamine as well as other acid stimulants. As there is some delay in onset of efficacy, they tend to be used for chronic conditions, whereas H2 blockers are used more acutely due to having more immediate effects. As with H2 blockers, there are concerns with interactions with other medications. Drugs in this category include Aciphex (rabeprazole), Nexium (esomeprazole) Prevacid (lansoprazole), Prilosec (omeprazole) , Protonix (pantoprazole) and Dexilant (Dexlansoprazole) 
 
There are issues with the ongoing use of any of these medications. Stomach acid is required for digestive function. Acid strengthens the gastro esophageal sphincter, thus helping to prevent reflux. Acid activates the digestive enzymes, facilitating the digestive process. Acid enables the absorption of major nutrients including calcium, magnesium, B6 B12, folic acid and Vitamin D.  Acid maintains a stomach environment not conducive to the growth of certain bacteria.
 
Without stomach acid, there is increased susceptibility to pneumonia, an increase in osteoporosis with resultant hip fractures, other deficiency diseases, and lower bowel problems. The lack of stomach acid weakens the gastro esophageal sphincter allowing acid to go where it should not go, causing burning and digestive distress.
 
Foods that increase heartburn
Working with heartburn, weaning off drugs, and stabilizing digestion can be quite a journey. Pressure on the abdomen, as in pregnancy, can add to heartburn misery by causing mild to severe hiatal hernias, which must be addressed to lessen the pain. Sleeping on a slant can be helpful. Foods and drinks such as citrus fruits, tomatoes and spicy foods, and alcohol and caffeine can aggravate the problem, as can smoking cigarettes, so these are best noted and avoided if warranted.
 
Initial naturopathic treatment options may include the use of deglycyrrhizinated licorice (DGL) to prevent the development of ulcers, to protect and heal the tissue and to soothe the pain. The gastro esophageal sphincter can be strengthened with probiotics, apple cider vinegar, and/or digestive bitters, which also stimulate enzyme production. Gamma oryzanol from rice bran oil tones the sphincter.  Other individualized options may also be necessary to calm that digestive fire.

 Eliminate Your Belly Ache
Ideas from My Personal Journal

When I was younger I had a lot of digestive problems and thought it was my lot in life to suffer through them.  I had problems with dairy, cruciferous vegetables and beans (OK, I'm not the only one). I dreaded what was going to happen when I got older because there was already a history of digestive problems in my family going back to, at least, my grandmother. But that was over 50 years ago and I am delighted to report that my digestion is normal. It's still hard for me to believe it.

So what did I do to achieve a normal digestion?

First, I began to notice what foods caused indigestion and eliminate them one by one. For example, I discovered that eating deep fried foods caused so much belly pain that it was no problem to stop eating anything deep fried. I didn't do it because deep fried foods are unhealthy (they are) but I just didn't want to suffer.

Just to be fair, there are some healthy foods that I refuse to eat like cucumbers because they repeat in my system for days afterward. We are all different and it just takes tuning in to your body to notice what is going on for you specifically.

 I lived in Japan for 2 ½ years, tried to eat more vegetarian, and I got sick. I was told that being a vegetarian was healthier than eating meat but that was not true for my body.

The best eating advice I have ever gotten has been from my own body.

And then there is the fact of what emotions do. I had to admit that I had an excitable personality and that eating when I was upset was a sure way to incite the belly pain.  But how to calm down?  After all, I'm a Cuban and that is not a "keep calm and carry on" cultural heritage!

The most important thing you can do to calm down is to breathe slowly and consciously. My own preferred method is to breathe into my belly for a count of 4 seconds, hold 2 seconds (for the oxygen/carbon dioxide exchange), then breathe out slowly through my mouth for 7 seconds. I do this 4X in a row and then my breathing slows down automatically.

I have even had a wristband created which says "BREATHE" on it which I wear and give to clients. This really works well and the reminder has even helped my body aches.

Another easy calming tool I use is EFT (Emotional Freedom Technique) or Tapping.

I teach EFT to every client in the first session and they leave knowing how to do it. EFT gets the body to calm down by directly addressing our ancient "fight or flight" response to stress.

When stressed, the body releases the hormone cortisol, which diverts blood away from your digestive tract, leaving you less able to digest. Well, that sure makes sense.
 
A recent study in the Journal of Nervous and Mental Disease, the oldest peer-reviewed psychology journal in the United States, found that EFT lowered the major stress hormone cortisol significantly more than other interventions tested.
 
These two tools have helped calm my system down more than anything else in my life. I can't recommend them enough.

Grilled or Broiled Peaches

½ organic peach per person, washed, pitted and cut in half(nectarines and plums work well too)

Dressing: 
¼ cup Peach Balsamic Vinegar
1/3 cup Blood Orange Olive Oil or Avocado Oil

Place all ingredients in a Mason jar, close and shake until well combined. 

Broiled:

Preheat oven to broil. Place peaches in a glass baking dish cut side down. Pour half the dressing over the peaches, turn them over and spoon dressing over each peach. Turn oven on to broiler and place under broiler 6 min each side or until soft and browned, not black. 

Grilled:

BBQ grill over medium heat. Broil or grill 6 minutes per side. Remove from broiler and pour remaining half of dressing over peaches. Serve in a bowl with a scoop of faux Vanilla Ice "Cream"!

Our thermography patients are understandably motivated to avoid the potentially detrimental effects of annual screening mammograms. Radiation, as we all know, is one cause of cancer. And compression of breast tissue can be painful and damaging to tissue.
 
However, when results on thermography are equivocal or high risk, or when there are observable tissue changes on self-exam or on physical exam, we do recommend anatomic screening to augment the physiologic screening that thermography provides. Mammograms are one option. The cost benefit ratio shifts when there is a need to clarify a given situation.
 
Lumps and bumps can sometimes be evaluated with ultrasound rather than mammogram. Ultrasound can reveal whether a mass is cystic or solid. As another option, MRIs may be more accurate than mammograms. However, insurance tends to approve MRIs in high risk situations, rather than for general screening purposes.

The product images on the package certainly look appealing, but how do you feel after you've eaten these foods? Sluggish, hyperactive, or left with a stomach ache? Researchers Develop Faster, 
More Accurate Test for Liver Cancer 
That Can Be Administered Anywhere

Summary:

A rapid portable screening test for liver cancer has been created that doesn't involve sending a specimen to a blood lab and cuts the wait time for results from two weeks to two minutes. This inexpensive test can be administered wherever the patient is, which will be valuable for developing nations with little access to hospitals.
