The Summary of eneBfits and Coverage (SBC) document is a handy reference. The SBC explains the benefits breakdown of the plan. For questions about the SBC, please contact the Sales team at 877.230.7615.  Needed codes, such as HMO02715 or PHA00821, can be found on renewal documents.
