Doctors Care Embraces Team-Based Care
Funding provided by Caring for Colorado Foundation and the Colorado Health Foundation, is paving the way for Doctors Care to implement team-based care practices that will strengthen the delivery of integrated care for the organization.

In addition to providing medical care, in the last two years Doctors Care has added behavioral health services, care coordination, navigation services and dental hygiene services. Goals for this project include improving coordination of care and services to eliminate gaps and/or missed opportunities for patients and clients. Activities include standardizing processes, procedures and workflows to increase efficiencies and enhance roles and responsibilities for staff and volunteers.

According to Clinic Director Sue Covington, "We want to make sure the patients we serve receive the maximum benefit from all the services Doctors Care provides. Achieving our team-based care goals will improve patient, staff and provider satisfaction and solidify continuity of high quality care, resulting in better outcomes for all our patients."
Dental Services Integration On Track
Integrating dental services into the Doctors Care Clinic is still in its early stage, but great strides are being made every day. Thanks to combined efforts by providers, medical assistants and front office coordinators, more patients are becoming introduced to dental hygiene services by our Dental Hygienist, Doreen Swatloski.

According to Doreen, "The team approach of integrating dental preventive services is proving successful as a friendly and convenient way of addressing a great need for Doctors Care patients." The opportunity to meet patients in the medical exam rooms is a great way to introduce dental preventative services. These efforts are paying off as more patients are getting a chance to schedule convenient same day dental services, along with their medical appointments.

Volunteer Spotlight -
Edward Simmons

Edward spent the summer volunteering at Doctors Care in the Health Navigation program. According to Edward, he enjoyed his experience working in this area as it provided him and his mother Norelis Cadena, also a volunteer, the opportunity to do outreach to people who most need help. "We get to go to food banks and really awesome events, which is fun," says Edward.

What they have learned, he says, is that it is important to reach out to the people in need, as it is difficult for them to ask for help. Edward has had to overcome his shyness and found that reaching out has become easier with practice. While now back in school, Edward plans to continue volunteering with Doctors Care during special events. He enjoys his time with Doctors Care and appreciates the opportunities he's had to learn and to reach out and help those in need.
