This month, we're featuring two projects that are forming multi-sector collaborations to promote healthy behaviors and prevent chronic diseases in their communities.
BUILD: Addressing Healthcare's Blindside in Albuquerque's South Side
Adelante Development Center, Bernalillo County Office of Health and Social Services, and Presbyterian Healthcare Services
are partnering to
expand access to healthy food and equip healthcare providers to refer patients to a service hub to address social and economic needs in Albuquerque's South Side. The project is developing a referral tracking system that connects patients to a membership rewards program that incentivizes healthy behaviors.
CHP: Minneapolis Heart Institute Foundation
Minneapolis Heart Institute Foundation works to improve the cardiovascular health through innovative research and education, and is well-versed in cross-sector collaboration. As a CHP Subject Matter Expert Community, they will share lessons learned on the development, implementation, and evaluation of Hearts Beat Back®: The Heart of New Ulm, a 10-year community-based initiative to reduce heart attacks in New Ulm, MN.
