Exploring the Intersection of Clinical and Social Determinants of Health Data

At a recent HealthDoers peer-to-peer event, 75+ stakeholders met to discuss promising approaches for combining social determinants of health data with traditional health care data. A new  All In blog summarizes key steps that can be taken at the national and local levels to advance this work.
 

Building the Business Case for Community Partnership

A new Advisory Board white paper explores four steps for building effective community partnerships to extend care team reach, engage consumers, and improve cost and quality. It highlights themes and lessons learned from BUILD Health Challenge sites.

Chicago DASH Grantee Recognized for Top Innovation in Government

Congrats to the Chicago Department of Public Health, whose predictive analytics system for food inspections has been honored as a Top 25 Bright Idea from Harvard University! The system leverages public data to identify Chicago restaurants most likely to face health code challenges, helping to prioritize inspections and prevent food borne illnesses.


Project Spotlights

This month, we're featuring two projects that are forming multi-sector collaborations to promote healthy behaviors and prevent chronic diseases in their communities. 

BUILD: Addressing Healthcare's Blindside in Albuquerque's South Side

Adelante Development Center, Bernalillo County Office of Health and Social Services, and Presbyterian Healthcare Services  are partnering to  expand access to healthy food and equip healthcare providers to refer patients to a service hub to address social and economic needs in Albuquerque's South Side. The project is developing a referral tracking system that connects patients to a membership rewards program that incentivizes healthy behaviors.

CHP: Minneapolis Heart Institute Foundation 

Minneapolis Heart Institute Foundation works to improve the cardiovascular health through innovative research and education, and is well-versed in cross-sector collaboration. As a CHP Subject Matter Expert Community, they will share lessons learned on the development, implementation, and evaluation of Hearts Beat BackĀ®: The Heart of New Ulm, a 10-year community-based initiative to reduce heart attacks in New Ulm, MN.

  • Culture of Health Leaders ProgramApply by Feb. 15 - This three-year RWJF leadership development program offers an annual stipend for up to 50 participants from diverse sectors who are integrating health into their work.
  • Emerging Leaders in Public HealthApply by Feb. 20 - Apply to receive leadership coaching and a grant to develop and implement a project designed to deliver a new model of public health for your community. 
  • The BUILD Health Challenge: Apply by Feb. 21 - BUILD is funding 18 new sites to implement bold, upstream, integrated, local, data-driven solutions aimed at influencing the social determinants of health.
