Continue the dialogue with referral physicians.
Thorough documentation starts with physicians. Talk with referral sources and agree on a plan to ensure you obtain the documentation you need. Our ICD-10 webinar serieswhich addresses physician documentation, is a good reminder and may help you further the discussion. 

Keep up detailed clinician documentation. Document those details needed to accurately code in ICD-10. Don't forget to include items such as therapy considerations for home health and a specific terminal diagnosis for hospice.   


Ensure your internal quality assurance process is sound. Now is the time to perform a thorough audit of your process and schedule training to address areas for improvement.
