Hammond-Henry Hospital to Open Clinics in Cambridge a nd Annawan in January

Hammond-Henry Hospital will open two new clinics in January 2018. Hammond-Henry Regional Health Partners will begin accepting patients in Cambridge on January 8, and the Hammond-Henry Annawan Walk-In Clinic will begin accepting patients January 15.
  
"We are excited to announce that we are adding two new clinics to better serve our patient families with same-day care availability," Mark Kuhn, CEO at Hammond-Henry Hospital, said. "We are pleased to offer care not only to those within the hospital but to the communities we serve as well - through all our area clinics, including Geneseo, Kewanee and Wyoming, as well as the Urgent Care Clinic in Colona and the two new clinics in Cambridge and Annawan."
  
Hammond-Henry Regional Health Partners, Cambridge Opens January 8
Located at 106 N. East Street, the Cambridge clinic will offer same-day and by-appointment care by the team of Hammond-Henry Regional Health Partners. Heidi Vorac, APN, is returning to the Cambridge community part-time. The newest RHP team member - Lindsay Tapia, APN - will see patients at the Geneseo and Cambridge clinic locations. Hammond-Henry Annawan Walk-In Clinic Opens January 15
Located right off I-80 at 110 S. Depot Street, the new Walk-In Clinic will deliver reliable, acute care services to the community - with no appointment necessary. Led by Medical Director Donald Ford, MD, the care team will include: Katie Sowder, APN; Treena Adams, APN; Amanda Ryan, APN; and Kathy Versluis, APN.
Hammond-Henry Long Term Care Living Center Continues Award Streak

Hammond-Henry's Long Term Care (LTC) Living Center has again received the highest possible overall rating of five stars from The Center for Medicare and Medicaid Services (CMS). CMS annually updates their official government website Nursing Home Compare Five-Star Quality Rankings based on rating nursing homes by three primary categories - health inspections, staffing, and quality measures to develop an overall star rating. This rating system continues to include new quality measures covering successful discharges, emergency visits and readmissions to the hospital. 

CMS determines their star ranking using regular inspections and data compiled from nearly 800 Illinois long-term care facilities serving approximately 100,000 residents.  Less than ten percent (10%) of the facilities achieve five-star status each year.
