Dear 7th Grade Parent/Guardian:
The purpose of this email is to inform you of immunization requirements that will impact rising 7th   grade students. Students entering 7th   grade are now required to have one dose of Tdap (tetanus, diphtheria, pertussis) vaccine and one dose of meningococcal conjugate vaccine for individuals who have not already received those vaccines.

Please contact your health care provider or local health department if your child needs these vaccines. For additional information about the vaccines and diseases they protect against visit the Centers for Disease Control and Prevention’s website at www.cdc.gov/vaccines/who/ teens/index.html.

You will have 30 calendar days from the first day of school, September 27th, to provide the required up-to-date immunization record for your child. If proof
of vaccination is not provided by the end of the 30 calendar day period, your child will be suspended from school until they provide proof
of immunization as required by law.

Kindergarten Health Assessment

Health Assessments  communicate the health concerns and information from parents and physicians for all children entering public schools for the first time in North Carolina. Health issues that affect learning are addressed.

Every child in this State entering kindergarten in the public schools shall receive a health assessment. The health assessment shall be made no more than 12 months prior to the date of school entry. No child shall attend kindergarten unless a health assessment transmittal form indicating that the child has received the health assessment required by this section, is presented to the school principal. The parent, guardian, or responsible person shall have 30 calendar days from the first day of attendance to present the required health assessment transmittal form for the child. Upon termination of 30 calendar days, the principal shall not permit the child to attend the school until the required health assessment transmittal form has been presented.

You may download the kindergarten health assessment here for your child's doctor to complete. Please return the health assessment to the front office by September 27, 2018.
