Letterhead
Ohio and Indiana Requiring Taxpayers to Complete Identity Confirmation Quiz for Refunds

To Our Clients and Friends:

In order to combat identity theft and tax refund fraud, some states are beginning to confirm identity by sending a letter requesting taxpayers to take an online quiz in order to confirm their identities. Indiana implemented this recently, and now Ohio is following suit.

If you receive such a notice in one of these states, you should be aware that your tax return will not be processed unless you go online and complete the identity confirmation quiz. For taxpayers who do not own or use a computer, the quiz can be completed by phone. The quizzes for Indiana and Ohio can be found here:
It's always important to keep an eye out for communication from your state's tax authority. However, delaying your response to this correspondence will delay your refund.

If you receive a letter requesting that you take an identity confirmation quiz and did not file a return, or have someone file a return on your behalf, the quiz provides an option to report fraud.

These notices are real, so do not disregard them!
