Dear Project Participant,

IPRO End Stage Renal Disease (ESRD) Network of the Ohio River Valley supports the national initiative to increase the number of patients on the transplant waitlist. To accomplish this goal  the Network will work with 30% of the facilities in the Network service area to demonstrate a 10% increase in the number of patients wait-listed based on data available in October 2018. Each facility in this QIA has received information on the activities they are supposed to undertake to reach this goal.  Patients who are medically ineligible are excluded from this project
The Network has begun to work on some small tests of change working with individual transplant centers and surrounding dialysis facilities to identify and overcome barriers  and establish processes to improve the transplant wait list process. Once strategies have been compiled they will be shared across the Network community.  
    
Facility Project Lead To Do's:
  • Review your patient census monthly to determine which patients are medically ineligible for transplant, and document this with a note in the medical record 
  • Overdue:  Complete the Network e-mailed survey identifying ineligible versus eligible patients in the excel spreadsheet provided, upload that spreadsheet to the redcap email and return the information to the Network as soon as possible. Do not send the completed spreadsheet via personal NW email this is a HIPPA violation
  • Overdue:  Non-LDO facilities must also complete the second tab in the spreadsheet identifying where their eligible patient census currently falls in  the seven steps to transplant as outlined by CMS.
  • Print educational materials for your Patient Ambassadors to distribute, see below
  • Review progress with your Patient Ambassador and report the interventions in QAPI
  • Assist Patient Ambassadors in providing local transplant contact information to all interested patients
  • Transplant poster contest:
      21 facilities participated in the Transplant Awareness Poster contest.  The submissions are currently being reviewed by a patient panel and a winner will be awarded soon.  We applaud your creativity
    and dedication to educating your patients on transplant.
  • Compile transplant success stories from your facilities patient population and share them with your patient ambassador and the Network
  • Discuss with your patient ambassador what were his or her successes and determine how we can get more patients involved in the future. Share these findings with the NW
  • Coming soon:
      We are asking for your input with a midpoint project data collection to assist the NW in identifying project successes and barriers. This will be sent via email to the project leads in the form of a redcap survey
Patient Ambassador To Do's:
  • Distribute NCC materials on transplant, see educational resources on the left.
  • Provide interested patients with contact information for the local transplant centers
  • Distribute the Network Transplant Criteria information 
  • Provide National Kidney Foundation Peer Mentor Information
  • Share transplant success stories from your facilities patient population and the stories provided by Your Life Your Choice 
  • Discuss your feelings on the patient ambassador program with your project identifying changes and improvements you would like to see.  Discuss how we can get more  patients involved with their care
  • Work with your project lead to schedule a time you can report on your efforts at the facility quality team meeting.
