Best Practices for Assisting Homeless Individuals

The title for this training will focus on Engagement and  Best Practices for Assisting Homeless Individuals  in our Community and will be facilitated by the Catholic Charities Homeless Outreach Street Team (H.O.S.T)

Our very own H.O.S.T staff members will present an informative training and discussion on best practices they use to assist those experiencing homelessness. This training will be a broad overview on how to interact with homeless individuals, both within our agency and within our community. The H.O.S.T team will give helpful insight on ways to interact and speak to homeless individuals. Catholic Charities staff members will also give tips on how to de-escalate an angry or upset homeless individual, when you should/shouldn't give money or engage, and best ways to serve them (give them a hand-up, not a handout). 

Catholic Charities Learning Series training's are open to all!

They are a great way for you to meet other staff and volunteers of Catholic Charities, to learn more about all of our programs and also, how they connect with the work that you do.   This is a training that you do not want to miss!!

Following up on the announcement you received last week, Community Foundation Sonoma County and Napa Valley Community Foundation have engaged the Center for Effective Philanthropy (CEP) to survey nonprofit organizations in Sonoma and Napa counties about the impact of, and their response to, the October 2017 Northern California wildfires. 

The Community Foundations are seeking to learn about your fire response, community needs, and your organizational needs. As a nonprofit leader, your input is crucial to help inform fire recovery efforts.

Financial Coach
The Financial Stability program trains volunteers to conduct one-on-one financial coaching to individuals, families, and youth. Volunteers will assist individuals in developing the skills needed to build assets and economic stability that will increase their ability to become self-sufficient. Financial Coaches offer continued support to clients so that they can meet their goals and tackle challenges. You will meet with clients one-on-one and provide assistance with goal setting, planning, problem-solving, and brainstorming. The goal is to help the client reduce debt, improve credit score, build assets, and accomplish goals through behavior change.  Finance experience welcomed, but not required.
 
Donation Delivery Assistant
Catholic Charities of the Diocese of Santa Rosa is in need of volunteers to assist at our Restyle Marketplace. Restyle is the organizations thrift store. We are in need of volunteer to go out on truck deliveries with a staff member and assist him with donation pick-ups. Donation delivery pick-up days are typically on Tuesdays and or Fridays from 10:00am-2:00pm Will require lifting of 50lbs or more with assistance.
 
Alzheimer's Shaw Day Center Volunteer
Catholic Charities is looking for volunteers to assist in our Shaw Day Center for Memory Care. Our program runs Monday - Friday from 9:00am-3:00pm, and scheduling is flexible. Volunteers work in our day center and assist in activity programming for people with dementia, assist staff with clients during meals, and more! As a volunteer, you will be part of a strong community that provides essential supports to individuals with dementia and their families. Volunteers must complete a background check and pass a TB Test.

Position Summary
The Community Advocate is stationed at Sonoma Works and is responsible for providing assistance and resource information to victims of domestic violence. The Sonoma Works Advocate acts as an intake worker for the Season of Sharing fund, screens applicants, and assists with the completion of forms.

This is a high-profile, public-interaction position requiring a high degree of compassion, professionalism, and a constant awareness that the employee is often the first non-law enforcement contact the victim encounters. The Community Advocate is truly an 'ambassador' for the organization. Public presentations about domestic violence issues are occasionally required. The Community Advocate must be available to be on a rotating on-call schedule, and be responsive to phone calls and to immediate law enforcement advocate requests.

Job summary
Under the direction of the Therapy Program Manager and our team of Clinical Supervisors, the MFT/ACSW Associate provides direct clinical services to women, men, children, and families who have either experienced or witnessed domestic violence or who have experienced or are at risk of abuse. Individual, family, and group work may be performed depending on intern skill set and interest and on assignment availability. This position includes: providing direct psychotherapy treatment, performing client psychosocial assessments, developing treatment plans and guiding the treatment process, developing exit plans, providing psychological and emotional support to clients, facilitating client support groups, adhering to client confidentiality standards, training in trauma informed therapy and participating in individual as well as two hours of group supervision each week. In an attempt to foster greater client empowerment, all direct psychotherapy treatment duties are to be carried out through a client-driven model through which client input heavily shapes treatment plans and goals. This position requires a minimum a one year commitment that may be extended at the agreement of both parties.

Prerequisite Qualifications

* Experience working with children, adolescents and families preferred.

Jewish Family and Children's Services (JFCS) is looking for a Disaster Recovery Case Manager to provide long-term case management services to fire survivors served by Rebuilding Our Community (ROC) Sonoma on behalf of Jewish Family and Children's Services. The Recovery Case Manager (RCM) will be responsible for providing services to clients who were affected by the 2017 Fires as they rebuild their homes and their lives. 

Recovery Case Management activities will include screening, assessment, planning, goal setting and supporting client access to resources, both financial and community wide. RCM work with ROC Sonoma to help clients' access community resources, Unmet Needs funding and other assistance to obtain long-term housing. 

PRIMARY AREAS OF RESPONSIBILITY
  1. Work with RCM Team and Committee to triage and screen all potential clients needing recovery case management.
  2. Maintain the highest levels of confidentiality regarding client information, sharing it only as agreed upon by the client and as evidenced by a signed release form.
  3. Performs outreach to identify persons in need of services and referrals related to disaster recovery.
  4. Facilitates credit counseling and financial education, guiding participants with the development, implementation and monitoring of individualized opportunity plans
  5. Stays current on resources for clients to insure the best success in meeting their goals.
  6. Performs interviews via phone or in person to provide prompt and responsive initial assessment of applicant needs; conducts comprehensive, individualized, strengths-based, trauma informed and culturally-responsive review of each client's disaster recovery needs and related available resources.
  7. Engages each client to cooperatively participate in the development, implementation, and on-going review of an individualized disaster recovery plan related to needs surfaced in assessment.
  8. Provides education and information to assist clients in effectively accessing the resources available in accordance with the sequence of assistance for disaster recovery.
  9. Organizes and prepares information and reports for meetings, briefings, and conversations with clients, staff and external partners.
  10. Acts as an advocate for and liaison between clients and other resource providers and organizations; collaborates with services providers, governmental and non-governmental agencies and other organizations and businesses to coordinate services for clients. Travel on a regular basis to provide face to face services to clients.
  11. Monitors client progress toward recovery goals; records relevant information using standardized forms and entering them into ROC Sonoma's Client Tracking System in a timely manner.
  12. Work closely with all internal program staff to provide relevant services, as well as with partners in the RCM Committee and partner agencies in the larger ROC Sonoma Long Term Recovery Group.
  13. Other related duties as assigned.
QUALIFICATIONS: Education, Experience and Skills Required
  • A minimum Bachelor's Degree and two years of successful experience in similar position or related field is preferred; enrollment in related classwork or related field such as community health, public health, social services, or psychology preferred.
  • Bilingual (English/Spanish) helpful.
  • Strong written and verbal communication skills; organizational, conflict resolution and computer literacy.
  • Strong interpersonal skills with people of all ages
  • Passion and enthusiasm for the mission of JFCS and its clients.

Intro: The County of Sonoma Probation Department is seeking a highly skilled welder to teach at the Sonoma County Probation Camp. The Welding Instructor at the County Probation Camp in Forestville instructs adolescents who are under the jurisdiction of the Juvenile Court. This position trains and supervises a group of youth offenders who perform vocational work for a variety of government and private agencies, as well as oversees the Welding shop; manages welding product orders, assists in the carpentry shop, and transports youth offenders.

Intro: The Family Advocacy Support and Stabilization Team (FASST) works in conjunction with Sonoma County Behavioral Health case managers, who have identified youth who are at risk of hospitalization or have recently returned to their home after a 5585/5150 hospitalization. FASST clients are high risk of relapsing.
The FASST clinician will provide 1-2x weekly direct services to the client caseload of 10-15 clients, and be comfortable manning a Spanish Crisis/ warm-line. The FASST clinician will collaborate with the county case manager and will attend the 1x weekly FASST meeting at Sonoma County Behavioral Health. The FASST clinician is experienced in providing psycho-education and coaching parents on how to manage mental health crises, will have experience in assessing clients and family systems for safety and risk behaviors, and is an effective and proactive communicator. Services are provided in the community and home settings of the client. The FASST clinician is expected to drive their own vehicle in order to perform the duties of this position. Bicultural preferred, Bilingual (English/Spanish) is required. Flexible schedule is available and possibility of working a weekend day in lieu of a weekday is available. Supervision toward MFT and/or LPCC is available.
RESPONSIBILITIES
  • Conduct mini assessments and completes treatment plan 
  • Engage primary caregiver, youth and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement
  • Clearly and concisely document treatment efforts via electronic medical record keeping system (MediCal billing through Tier) meeting expected organizational timelines 
  • Maintain 65% billable productivity throughout each month
  • Collaborate with all relevant systems and key participants within each system (Sonoma County Behavioral Health).
  • Provide direct clinical treatment using methods consistent with Evidence Based Practices (Triple P, DTQI, Crisis Interventions, CBT) Participate in all trainings, supervision, and consultation activities.
  • Provide intensive therapy and case management services to case load between 10-15 High Risk, Specialty Mental Health clients who live in at home or foster homes

Intro: The full-time, non-exempt, Community & Fire Recovery Bi-lingual Case Manager Level I, II & III provides case management services to clients of Catholic Charities. Case Manager is responsible for providing client assessment to clients who were affected by the 2017 Fires; the purpose of this position is to provide long term support to families as they rebuild their homes and their lives.

Case management activities will include determining appropriate financial assistance including but not limited to access rental subsidy assistance to obtain long-term housing; facilitation of credit counseling and financial education, guiding participants with the development, implementation and monitoring of individualized opportunity plans, providing assistance with finding and accessing appropriate related community resources and assisting participants with identifying and overcoming any barriers to access services and achieving goals. Level I, II to III starting hourly rate: $18 to $22
