How do you transfer patients between facilities?

There are so many important parts to a transfer so the key is to be prepared.  You can do this by reviewing the inter-facility transfer toolkit and find out what else you can do to prepare.  Feel free to use the samples provided in the toolkit at your facility.


Where do you start?

1. Evaluate your current inter-facility procedures.

2. Review the EMSC inter-facility toolkit and decide what elements your department might add to improve transfers.

3. Implement new policies/procedures.  This can be daunting so take one or two measures at a time.

4. Consider how the processes you are developing will be sustainable.  How will these processes continue long term? Consider how your department make sure the appropriate documentation/checklists are stocked or that your new policies are a part of new employee training and ongoing training.
