MSPPS Presents at NYS Project Approval Oversight Panel & Receives News of Full Achievements in DY1, Q2 

Downstate PPSs were invited to present at the New York State Project Approval Oversight Panel to share updates with  DSRIP leaders from the New York State Department of Health, community/policy activists, and key stakeholders.

At the start of the event, announcements detailing each PPS's level of achievements were provided. MSPPS received news of its full achievement for all project requirements in DY1, Q2 and was awarded $4.3M.  A copy of our second quarterly report can be found here, along with a copy of scorecard that can be found at this link.  

MSPPS had the opportunity to share key highlights. Jill Huck, Director of the MSPPS Project Management Office, and Edwidge Thomas, Clinical Director of MSPPS, discussed our work in behavioral health integration, overall engagement with community based organizations, and our provider relations strategy.

Partner Spotlight:   Bailey House Offers Full Complement of Care, Key to DSRIP Efforts in Care Coordination

Bailey House offers free food pantry bags to patients. 
On Wednesdays, you might see the line out the door at Bailey House wrapping around 121st Street from Park Avenue in East Harlem. 

It's a food pantry community day, a day in which Bailey House distributes free food pantry goods for its clients and members of the surrounding community who may not have access to food.

"Sometimes it's a decision between putting food on the table and buying a Metro Card to go to work," said Denise Arzola, Senior Vice President of Behavioral Health and Operations. "For some patients, we're talking about the decision between buying medication and putting food on the table."

The food pantry is one of several ways that Denise and the team at Bailey House have focused on providing resources to community members who face barriers that could be detrimental to their health.

Bailey House began as an agency in 1983 answering to the needs of their community when the HIV/AIDs epidemic swept the nation. In the past few decades, they have learned that HIV/AIDs patients need more than access to medical tests. They need patient navigation, housing support, medical care coordination, access to behavioral health treatment, peer-to-peer support groups, health education, and independent living skills training.

Bailey House's CEO, Gina Quattrochi, joined the organization in 1991. Under her 25 years of innovative leadership, the agency has gradually expanded its reach to provide the full complement of support patients require to get back on track. Today, with the help of a centralized intake coordinator, these services are offered as a part of the health care journey at Bailey House for both HIV-positive patients and patients struggling with other chronic physical diseases, mental illness, and substance use.

Bailey House's services have also empowered their patients, giving them a second chance or an opportunity for rediscovery - working towards improved quality of life.

Denise shared a story of one of her patients who came to Bailey House and sat in the corner of the client lounge for weeks and perhaps even months at a time. He did not talk to anyone, did not acknowledge anyone, and all he wanted to do was sit there. Slowly as time went by, he grew comfortable. He began to participate in support groups, join classes, and even speak with peers and staff members. And then one day, Denise recalls being at a rally and was asked to find a peer who could speak to his or her experience. To much surprise, he raised his hand and stood up at the front.

"I couldn't believe it because the next thing you knew, he was leading the protest," said Denise. "It was almost as though he had found his voice."

 DSRIP, which is the ability to capture and understand  the full picture of the patient and to connect patients to needed services and resources addressing both physical and behavioral health. And now through DSRIP, Bailey House can connect patients to services and resources beyond their organization and across the local community. 

"DSRIP has really given us an opportunity to re-prioritize and think about ways to make our work more impactful," said Lauren Pallies, Vice President of Integrated Health and Operations. "We have been able to meet with partners we hadn't met with before, and to talk about ways to collaborate."

Bailey House actively participates in 2.c.i, 4.c.ii, and the Care Coordination Cross Functional Workgroup. As they continue to expand their services, they are excited to work with other partners to build a full complement of care for patients among their communities.

Partner Perspectives to Impact Future MSPPS Efforts

This week MSPPS began its multi-phase Process Mapping Session initiative focused on gathering partner insights to help with clinical and operational strategies for PPS projects DY2 to 5. 

During these project-specific sessions, partners discuss current state clinical processes and patient flows.  Partners across all provider types, including CBOs, and with varying levels of program implementation maturity  have been invited to these half-day in-person sessions.  Insights gathered will  help determine each project's future state model. 

The sessions will also initiate  partners' future state  feasibility assessments, inform partners' performance-base d roles and responsibilities as well as determine the MSPPS work stream  (IT, Contracting, Workforce, and Reporting) requirements across all  projects. The sessions also aim to remove traditional silo project approaches, by interconnecting project work flows to determine key dependencies and commonalities among projects. It will  also facilitate a more informative design of Project 2.a.i. - Integrated Delivery System.

Care Coordination, Ultimate Goal for MSPPS Call Center

MSPPS is developing a call center to help patients navigate care within the MSPPS network. As we receive patient level information from New York State Medicaid, MSPPS  will utilize this data to better understand the care patients' receive and their journey across the health care system. This data will inform the call center in its assistance with the PPS'  care coordination efforts.

Currently, the call center is still in early stages of development. A soft launch is scheduled for Wednesday, February 10th. In addition to preliminary care coordination efforts, the call center will initially focus on helping patients understand DSRIP and the Medicaid opt-out letters, which will continue to be distributed to Medicaid members beginning in mid-February.

Development of PMO Database & Partner Portal 

MSPPS has collected an abundance of data. This information has been key to meeting DSRIP milestones from the State and informing strategies with different MSPPS work streams.

The Project Management Office (PMO) has been working diligently on aggregating, organizing and managing this information through a new business solution, Salesforce.com. Through this process, the PMO will better understand each partner's organization's profile, patient reach, interests, and much more. This will key to improvements to current partner engagement efforts. 

Additionally, Salesforce.com will help drive the development of a partner portal. Partners would be able to: 
  • Update their contact information and partner profile 
  • View other partners' profiles
  • Access MSPPS announcements and any partner exclusive information 
  • Review contract metrics specific to their organization
  • Download and complete relevant requested documents 
  • Sign up for pertinent events, webinars, and sessions 
We are excited to use this data in a way that is not only meaningful for fulfilling State milestones, but as a way to open lines of communications across the network

Financial Health/Value Based Assessment
Distribution: Week of February 1st

Deadline: February 2016

The survey will assist the PPS assess the network's current financial health as a means of developing a financial sustainability strategy to address key issues. This will help the PPS achieve a key milestone for DY1 Q4.

Deadline: Mid- March 2016

The survey will be distributed to all partners and data will be collected by a third-party vendor, the Center for Health Workforce Studies (CHWS) to protect sensitive information. Only aggregate reports will be shared with the PPS and NYS. Please note that the completion of three MSPSS surveys will affect partners' payments for Performance Period 2 in DY1 - (1) Cultural Competency & Health Literacy, (2) Financial Health/Value Based Assessment, and (3) Workforce Assessment.

Demonstration Year 1 (DY1) Amendments Distributed; Next Steps
 
All DY1 payment eligible partners have received a DY1 Amendment Packet through Adobe Echosign. 

By February 29th, partners should complete and return:
  • The DY1 Amendment Packet through EchoSign
  • Complete and return the PP1 reporting scorecard and W-9 via e-mail 
Partners can expect to: 
  • Receive a follow-up e-mail with a copy of the webinar slides and recording from the January 27th/29th DY1 Amendment Update & Reporting Webinar
  • Hear from project managers regarding quarterly project status webinars for the week of February 22nd
  • Meet and engage with project managers and other partners during project group sessions at the February 25th MSPPS Town Hall
