MSPPS' Vision Presented at May Town Hall :
 
Late May, Medical Director, Dr. Edwidge Thomas and Senior IT Director, Patti Cuartas presented on the MSPPS strategy to organize MSPPS partners into community care hubs. The goal of this strategy is to offer patients access to a full menu of health services and care within geographical reach. 

Through MSPPS' technology solution, Community Gateway, MSPPS partners will be able to share data with one another and MSPPS. Data  that is collected via Healthix or the MS HIE will be aggregated into meaningful and useful information for partners to access in real time, thus better informing care and treatment decisions.  Various types of resources will exist within the partner portal, including: the "Patient 360" - a longitudinal view of the patients' visit history, resources on community based organizations, an online learning management tool (PEAK), the ability to make and receive referrals, the ability to view one's own performance as a partner, and more. As community hubs are established, MSPPS will develop technology solutions for patients to manage their own health.

Patti Cuartas, Senior IT Director, provided partners with an overview of the Partner Portal, a database that holds multiple resources for partners of the MSPPS.

MSPPS and MCO Contracting Update 

MSPPS took the lead by organizing a group of downstate PPS's in developing a common contract between the PPS's and Managed Care Organizations (MCO's). This contract applied to both the Equity Infrastructure Program (EIP) and the Equity Performance Program (EPP). Establishing this common contract language allowed for a more expeditious release of equity infrastructure (EIP) funds from the MCO's during the months of May and June. Drawing down the equity funding, which totaled approximately $25M of the PPS equity award, was a critical precursor to distributing allocations to partners for DY1. 
 
In order to draw down eligible EIP funds , PPSs must report to the MCOs supporting documentation on the achievement of select DSRIP activities. This includes expanding health homes enrollment, partnering with DOH-recognized tobacco cessation program, participating in DOH/and/or NYS efforts to end HIV/AIDs, and working on fraud deterrence and surveillance activities. MSPPS has begun incorporating these measures in current DSRIP work.

MSPPS partner, Amsterdam Nursing Home has been recognized as a top nursing facility in New York State. For the past three years, Amsterdam Nursing Home has been ranked in the top quintile in the Nursing Home Quality Initiative (NHQI), a quality and performance program that ranks nursing facilities based on quality, compliance, and efficiency.

DSRIP's Collaborative Benefits to Project 4.c.ii 

Matt Baney wears several hats. Not only is he the Senior Director for the Institute for Advanced Medicine at Mount Sinai Health System, but he has been a spokesperson and an advocate for DSRIP and Mount Sinai PPS.

Although there are challenges with DSRIP implementation, Matt acknowledges the program's ability to foster great partnerships across the city. "This is a unique and complicated opportunity, but ultimately I think we can make a change and have an impact upon public health," said Matt.

Through this experience, he has collaborated with several HIV care agencies and community-based organizations to improve HIV services in New York City. Most recently, he was able to share his experience with 4cii and DSRIP to faith-based communities in East and Central Harlem and at the New York Health + Hospital PPS.

We look forward to seeing him shine on stage at Washington, D.C when he presents at the 2016 National Ryan White Conference on HIV Care and Treatment.

Little Sisters of the Assumption Family Health Service Prepare for DSRIP Engagement with Other CBOs in their East Harlem Community Partner

This month, our Partner Spotlight Series focuses on community-based organization, Little Sisters of Assumption Family Health Service. LSA Family Health Service's mission is to promote the physical, emotional, educational, and spiritual dimensions of family health within and around the East Harlem area.

The Imp act of LSA Family Health Service 
LSA Family Health Service (LSA) has a 58-year history in East Harlem, and those who have interacted with  the community-based organization are still touche d and affec ted today by the genero sity and compassion it shows to the community.

LSA is excited to engage in DSRIP. According to LSA's CEO, Traci Lester, "LSA is ramping up to be an active participant in DSRIP. We are going through our own internal review and strategy planning to get our programs up to speed on DSRIP, so when this goes live we are ready." LSA wants others to be aware that they ar e "in it to win it" and are prepared to be successful with this endeavor.

LSA participates  in Project 2.b.viii, Hospital Home Care Collaboration Solutions,with  their  Certified Hom e Health Agency (CHHA), which focuses on pre-natal, post-partum, newborn, and pediatric assessments.  The ag ency is integral to the East Harlem Community, providing nursing for 100 patients at a time, about 2,500 hom e visits per year.

The CHHA is operated primarily through a referral process and physicians from 
Mo u nt Sinai have utilized LSA's services by referring their patients. Patients are also eligible through fax and phone-in referrals as well as walk-ins.

LSA's Involvement with DSRIP 
Employees of LSA are hopeful that DSRIP will allow them to transform and provide the community based organizations with great opportunities for which they would otherwise would not be eligible. 

Russell Nobles, Chief of Program Operations, said of DSRIP, "LSA's DSRIP Connection with Mount Sinai is extremely important to us as a CBO. Several of our programs are directly funded by Medicaid. We consider the restructuring of Medicaid and the delivery of services to be of paramount importance, from our economic stability to the way we function and organize ourselves. We want to be as much of a part of this process as we can possibly be."

LSA is also hoping to gain better connectivity among other CBOs with DSRIP. Being located in East Harlem allows LSA the opportunity to integrate with other CBOs in close proximity. 

Lester said of this integration, "We each have our own special area we focus on, and together it makes for a great collaboration among the agencies. It is really important to collaborate and to keep our fingers on the pulse of these changes as they're occurring."

Contracting Update - DY1 into DY2
 
MSPPS is wrapping up the contracting process for DY1. All PP2 evaluation will be complete by the end of August. Participating partners will enter DY2 contracting in the summer. Compliance Education & Survey for DY2 Payment
 
In mid-June, MSPPS partners received an e-mail with a link to the MSPPS compliance education slide deck and a request to provide information about the compliance operations at your organization. There were two requests attached: (1) A request for the latest compliance information and certifications; (2) An attestation confirming the receipt of this compliance education. This survey is due Tuesday, July 12th and excludes DY 1 Wave 2 partners and new DY 2 partners. If your organization is a part of these two groups, more information regarding surveys will follow shortly.

Tara Partovi joins us as the Senior Manager for Stakeholder Engagement. In this role, she will be building relationships with all of our partners within the PPS network mostly through site visits and ongoing engagement meetings and events. As part of the engagement process, Tara will be collaborating with Community Based Organizations, Consumer Advisory Board and with State and Local Agencies.

Ms. Partovi joins MSPPS with extensive experience in the healthcare industry, primarily in practice management and management consulting. Her most recent experiences include working for the Department of Surgery at Bassett Hospital and with the Public Consulting Group on implementing the Affordable Care Act in several states. Ms. Partovi received her Masters of Public Health at Drexel University with a concentration in Health Management and Policy.

Implementing New York's DSRIP Program 
 
The Commonwealth Fund recently published a report, Implementing New York's DSRIP Program: Implications for Medicaid Payment and Delivery System Reform*, which describes the tremendous undertaking of New York State to redesign Medicaid. The piece highlights the influence of this effort to transform health care systems in other states. The report was developed through interviews with federal and state officials, thought leaders from national health care organizations, and New York stakeholders involved in the "implementation of Medicaid Redesign."
