Panel Discussion At Drew University drew
The Rubino OB/GYN Group was invited to Drew University to be a part of an expert panel for women's health topics on February 21st as part of their of women's health week. Drs. Rubino, Abeshaus, Patel and Tang covered topics including contraception, vaccinations, STIs (sexually transmitted infections) and careers in women's health. 

The students were highly engaged and asked many important questions. We were honored to be their experts of choice.

Thank you to West Orange Mayor Parisi for helping us officially open The Rubino OB/GYN Group Mammography Center, an on-site, state-of-the-art, 3D breast center within our West Orange office. 

Our Center enables women to have annual well exams and mammography screenings during the same visit.

The Rubino OB/GYN Group was a proud sponsor of the Red Carpet event on March 1st at Canoe Brook to support Family and Children's Services, a non-profit behavioral health and social service agency with a priority in helping those most vulnerable.

As a sponsor, The Rubino ON/GYN Group's doctors and staff provided information on our practice and services while giving out branded giveaways. We were fortunate enough to share the step and repeat with Bradley Cooper. The event was captured in the video below:

There is often a lot of confusion surrounding contraception options for women. What are the best options, what is the easiest, what lasts the longest?

Dr. Saitta offers some insight into contraception and the various options available to women:

Contraceptive Options Available
There are a multitude of options available, from oral to IUDs:
  • Barrier Method: Condoms, sponge, diaphragm, spermicides
  • Oral contraceptive Pills: Combined estrogen and progesterone, and progesterone only
  • Alternative Hormonal Contraceptives: Xulane Patch, NuvaRing, Depo-provera
  • LARCs: Skyla, Kyleena, Mirena, Lyletta, Paragard, Nexplanon
Barrier Methods
Barrier methods are those that literally create a barrier to prevent sperm from reaching the egg of a female. They are only used when having sexual intercourse and include condoms, sponges, diaphragms and spermicides. 

Condoms: For men, options include condoms like Durex and Trojan, for women, there is Protetiv, Reality and FC2.  The efficacy of condoms is typically  95-98% with perfect use (78-82% with typical use) and it is t he only  contraceptive method that protects against STDs.

The sponge like Today's Sponge, has an efficacy of   80-91% with perfect use (76-88% typical use).

The diaphram is a shallow, dome-shaped cup made of silicone that you insert into your vagina to cover your cervix and keep sperm out of your uterus. It has an efficacy of 94% with perfect use (8 8% typical use). 

Spermacides include foams, creams and gels that use chemicals to stop sperm from reaching an egg. You put it in your vagina before sex to prevent pregnancy. They have an  85% efficacy with perfect use (71% typical use) when used alone.

Oral Contraceptive Pills
Oral contraceptive pills come in two options: estrogen/progesteone pills and progesterone only pills.
 
Combined Estrogen and Progesterone Pills
This option is available in 21-day packs or 28-day packs with 21-days of active pills and 4-7 days of placebos or "sugar" pills.
 
There are a v ariety of options available, mainly differing in dose and progesterone type included. There are some benefits and disadvantages to this option:
 
Beneifts:
  • Decreases menstrual heaviness
  • Decreases cramping
  • Can clear acne
  • Regulates irregular periods
  • Alleviates PMS symptoms
Disadvantages: 
  • Bloating
  • Weight gain
  • Break-through bleeding/irregular bleeding
  • Estrogen increases risk of blood clots/stroke in some populations
Progesterone Only Pills
These pills are available in  28-day packs with all active pills. They are the only  safe option for patients with medical conditions; for example, hypertension.

Emergency Contraception: Plan B
The "Plan B" emergency contraception is actually just a higher dose progesterone-only birth control pill. It is a vailable over-the-counter, therefore no prescription or ID is needed and there are no age restrictions.
 
You can take the "Plan B" pill up to three days after intercourse, but it is more effective the sooner you take it. It is not  an abortion pill - it will not work if you are already pregnant, and it won't harm an already existing pregnancy.
 
Side effects include  irregular bleeding, nausea and cramping.

Alternative Hormonal Contraceptives
There are three well-known alternative hormonal contraceptives int the market: 

Xulane Patch
The Xulane Patch is a transdermal patch (looks like a  band-aid) that transmits similar medications (estrogen and progesterone) to those in the pill but through the skin. It is c hanged every seven days, alternating between locations: upper arm, back, buttock and the stomach.

NuvaRing
The NuvaRing is a small, clear flexible ring (same size as a hair band) that contains estrogen and progesterone, which is inserted into the vagina. It is c hanged every 21 days, leaving the ring out for seven days in between.

Depo-Provera
The Depo-Provera is a progesterone only intramuscular injection given every three months. The noted risk with the Depo-Provera is it can cause  reversible bone loss and is currently not recommended to be continued for more than three years.

LARCs: Long-Acting Reversible Contraceptives 
LARCs are longer-acting and reversible contraceptives that include Nexplanon, p rogesterone containing IUDs (Mirena, Kyleena, Skyla, Liletta) and the hormone-free IUD - Paragard

Nexplanon implant is a very small rod inserted under the skin of a woman's upper arm to provide birth control. It is an invisible, progesterone-only birth control that prevents pregnancy for up to 3 years and is   99% effective. The most  common side effects include irregular bleeding. A doctor's visit is  required for insertion and removal.

Progesterone Containing IUDs (intra-uterine devices) include Mirena, Kyleena, Skyla and Liletta. The are p rogesterone-only birth control that last  3-5 years, depending on the device, and is 9 9% effective. The most  common side effect is irregular bleeding and a d octor's visit is required for insertion and removal.

Non-Hormonal IUD - Paragard
Paragard is a hormone-free birth control that lasts up to 1 0 years. It is 99% effective. The most co mmon side effect is heavier periods and more cramping and a doc tor's visit is required for insertion and removal.

Ultimately it is important to work in partnership with your doctor to determine the best type of contraception for your lifestyle.
An article in the February edition of OB/GYN News shared recent data  mom_kids_hug.jpg that "African American women with Alopecia have significantly higher odds of developing fibroids based on data from more than 400,000 women." 

Alopecia areata is an autoimmune disease in which the body attacks it's own hair follicles to cause hair loss. Alopecia can signal an increased risk in developing other conditions. The study, published in JAMA Dermatology, was initiated due to the correlation that both conditions are related to excess scarring and fibrous tissue deposition - however they didn't expect to find such significant results. 

As fibroids are often asymptomatic, the authors stressed the importance of doctors screening their patients with alopecia for symptoms of fibroids. Early recognition of fibroids is beneficial as it allows for timely treatment.
