Age-Friendly Communities (AFC)

Ontario is launching a new Age-Friendly Community Recognition Program to recognize communities and municipalities that are working to meet the needs of Ontario's growing seniors population by being inclusive, accessible and promoting the health and wellbeing of community members. Nominations for the Ontario Age-Friendly Community Recognition Program will open in fall 2017.

South East Regional Integrated Fall Prevention & Management Strategy

FASTlinks Maps have been developed as a system navigation tool for primary care health professionals in South East Ontario who are treating frail elderly patients with multiple co-morbidities.  Health care professionals in other care settings, older adults with functional impairment and their caregivers may also find FASTlinks useful.
