July 1 is the Deadline for Payment of Annual Accreditation Fee
Staff Contact: Ruth Crump, [email protected]

Annual Accreditation Fee invoices were emailed out during the week of May 1, 2018. This invoice came from Ruth Crump ( [email protected]) and was sent to the Billing Contact on record.

If you have not received an invoice, first check your spam folder to see if the invoice ended up there; if not, then notify Ruth at [email protected]; Ruth will email the invoice to you immediately.

The due date for the invoice is July 1. This applies to ALL programs that either hold CAAHEP accreditation or the CoAEMSP Letter of Review, or are working toward the LoR.

To avoid additional processing fees, payment is due on or before July 31.

NOTE: An Additional Processing Fee invoice of $500 will be sent the first week in August for past due Annual Accreditation Fees.

Avoid Unnecessary Fees and Delays!
Use the Correct Procedures for Making Personnel Changes, Additions and Updates
Staff Contact: Lynn Caruthers, [email protected]

Refer to these important guidelines to avoid unnecessary fees and delays!

  • Remember to let the CoAEMSP know of personnel changes, additions of new key personnel (such as Lead Instructors, Associate Medical Directors, or Assistant Medical Directors) AND basic office contact information updates for current personnel. CoAEMSP policy requires programs to notify the CoAEMSP of these changes/additions/updates within 15 calendar days to avoid a $100 fee.

  • Programs are required to answer each question on the 'General Information' tab of the self study based on their program. CoAEMSP staff review the personnel information submitted on the self study to ensure it matches the information in the CoAEMSP database. If the information does not match, the program will be notified via email, and the program will be required to submit documentation to rectify the discrepancy noted.

  • The only accepted method of changing personnel information is through the applicable CoAEMSP personnel change form provided on this this page of the CoAEMSP website. The CoAEMSP staff is NOT able to update records based on the information contained in a self study, progress report, annual report, email, or by phone.

  • If you need to change contact information for an existing key personnel (i.e. title, address, phone, email), you must use this form.

  • If you want to expedite the process of making a change in personnel, email all of the required documentation at one time, EXCEPT the official transcript,(required only for the Program Director and Lead Instructor) which MUST come directly to the CoAEMSP from the awarding institution to validate the individual’s highest degree. Be sure to review directions for submitting official transcripts here. NOTE! If an official transcript is already on file with the CoAEMSP, a new transcript is NOT required unless the individual has earned a higher degree.

  • All forms posted on the CoAEMSP website must be downloaded to the user's computer as a blank document; they are not web-based, so cannot be filled out online. Once the blank document has been saved, it can be opened and completed. The completed document can then be emailed to Lynn Caruthers at [email protected]. NOTE! Forms that are completed and emailed to Lynn without being downloaded appear BLANK when received!

  • The CoAEMSP does not accept forms completed by hand. All personnel documents must be typed and legible.

  • Be sure you are using the most current form! We are continually revising our forms in order to improve them. Always check the CoAEMSP website to be sure you have the latest forms and information. Do not hesitate to contact Lynn Caruthers at [email protected] or 214.703.8445 ext. 115 if you have questions.
