Complex Care Management Pilot in the Nephrology Setting

In October, the Value Oversight Committee awarded a grant to Maine Nephrology Associates and Maine Medical Center to pilot a collaborative, complex care management team in the nephrology specialty setting. The project adds RN care management, social work case management, and a nutritionist to the nephrology care team with the goal of helping stage 4 or 5 chronic kidney disease patients better manage their care.
