This months featured topic is Frailty and Older Adults. We have recently added more resources  about frailty on .

On April 21, 2017 Dr. Mary Tinetti gave the Keynote Address at the Canadian Geriatrics Society Annual meeting in Toronto. During that address she presented the GERIATRIC 5Ms - a simplified communication framework to describe core competencies in Geriatrics in a manner that those outside the field will be better able to understand and remember. We recently moved into the new Providence Care Hospital (PCH), which is the first publicly funded hospital in North America to fully integrate long-term mental health and psychiatry programs with physical rehabilitation, palliative care and complex care.  The hospital has been designed to be welcoming and inclusive of all people, no matter what age, ability or needs.  We are proud to call PCH our new home.

Advance Care Planning (ACP) & Health Care Consent (HCC) Education 
Michelle Howard of McMaster University will presenting at an upcoming webinar about the iGAP - Improving General Practice Advance Care Planning study where they have been collaborating with 20 primary care settings in Ontario, British Columbia and Alberta to develop, evaluate and implement ACP tools. Senior Friendly Hospitals
In April, Senior Friendly Hospitals highlighted dementia and delirium as well as a spotlight on NYGH for their novel way of making senior friendly care a corporate priority. The Delirium Care Network has developed a website featuring resources for healthcare providers, patients and families.To learn more click here .
FASTlinks Maps have been developed as a system navigation tool for primary care health professionals in South East Ontario who are treating frail elderly patients with multiple co-morbidities.  Health care professionals in other care settings, older adults with functional impairment and their caregivers may also find FASTlinks useful.
