NQP Partners Discuss Transitions of Care at PAC Meeting
More than 150 NQP partners attended NQP’s February Project Advisory Committee (PAC) meeting at Catholic Health Services of Long Island’s DeMatteis Center for Cardiac Research. The focus of the meeting was NQP’s progress pertaining to the Transitions of Care DSRIP projects, whose goal is to help patients successfully transition back into their communities after hospital discharge while preventing avoidable readmissions and emergency department visits. Innovative Smoking Cessation Campaign Targets New York Healthcare Providers

Northwell Health is promoting smoking cessation with a unique media campaign funded through a New York State Department of Health (NYSDOH) Health Systems for a Tobacco-Free New York grant. Targeted providers will receive a digital video mailer containing videos that relay the importance of speaking to patients about cessation and the success stories of organizations that have incorporated a smoking conversation into their practice. Catholic Charities Brooklyn and Queens Launches Programs to Bolster Community-Based Care
Catholic Charities Brooklyn and Queens (CCBQ) has implemented programs aimed at reducing behavioral health-related emergency department visits and hospitalizations, in alignment with NQP DSRIP projects 3.a.i and 2.b.iv.  Read more
Health Information Tool for Empowerment (HITE) Improves User Interface, Search Functionality
The Greater New York Hospital Association (GNYHA) has relaunched its Health Information Tool for Empowerment (HITE) website, www.hitesite.org, an online resource directory available to NQP partners and the public that contains descriptions and contact information of more than 5,000 New York health and social service organizations. Living Healthy Program Offers Chronic Disease
Self-Management Education
Through an evidence-based community workshop, Northwell Health's Living Healthy program aims to equip those with ongoing physical or mental illnesses with the skills and confidence they need to manage their conditions.  Read more
Value-Based Payment Patient Confidentiality Workgroup Report Open to Public Comment
The recommendations report of the Department of Health’s Value-Based Payment (VBP) Patient Confidentiality Workgroup has been finalized and opened for public comment.
