Northwell Health Creates EHR Tool that Supports Care Providers with PCMH Transformation
Northwell Health has created Clinical Snapshot, a point-of-care clinical quality dashboard that supports care providers with PCMH transformation. The tool displays key patient information in one place, on one screen within the ambulatory electronic health record, and has already been deployed to over 400 clinical users. Read more.
Catholic Health Services of Long Island Holds CBO Behavioral Health Summit
More than 30 Long Island community-facing employees received Master Trainer Certificates from a recent Cultural Competency/Health Literacy Train-The-Trainer workshop, hosted in partnership by Nassau Queens Performing Provider System, Suffolk Care Collaborative, and the Long Island Health Collaborative.  Read more.
Northwell Health Helps Extinguish Cigarettes at North Shore Towers and 10,000 Apartments throughout New York City
Cigarettes have been extinguished forever at North Shore Towers thanks to the collaborative efforts of Northwell Health, NYC Smoke Free, the New York City Department of Health, Queens Tobacco Control Coalition, community advocates, and the cooperative’s residents and board of directors. The initiative helps NQP’s efforts to meet DSRIP project 4.b.i, Promote Tobacco Cessation, objectives.  Read more.

NYSDOH Releases Whiteboard Videos: DSRIP in 2017; Myths and Facts of Value Based Payment
The New York State Department of Health recently released two new installments in the NYS Delivery System Reform Incentive Payment (DSRIP) Program whiteboard video series. One video focuses on the DSRIP Program in 2017, the other video focuses on myths and facts about the Value Based Payment initiative.  Read more.
