 Patient Access Hygiene and Surface Disinfection Audit tools 

 
Dear Project Participants,  
IPRO End Stage Renal Disease (ESRD) Network of the Ohio River Valley supports the national initiative on the reduction of blood stream infection (BSI) rates by 50% over the next five years. Cohort 1 and Cohort 2 will achieve a 20% reduction in their semi - annual pooled mean rate based on the data available in October of 2018. Currently, we have made great strides in improving BSI rates in our Network and have exceeded our CMS goal of a 20% improvement.Lets keep our eyes on the prize and work toward the ultimate goal of "0" BSIs.   
 
To assist you with compliance in the project we have provided you with the following monthly checklist:
 
CDC CORE Interventions To Do's:
  • All staff are educated on the CDC Core interventions and you have signed an attestation that you are using them in your facilities via our redcap survey.
  • Register and Attend the HAI-BSI National LAN events that will be announced for August. 
  • Cohort 2 only ( see facility list)- Complete the monthly recap survey identifying barriers and successes. 
  • Cohort 2 only- attend a webinar(registration) June 27th at 1pm outlining the patient ambassador Patient Access Hygiene and Surface Disinfection tools.
  • Print and distribute audits tools for the Patient Ambassadors to begin using.  
  • Become a member of the Making Dialysis Safer Coalition membership form
  • Coming soonWe are asking for your input with a midpoint project data collection to assist the NW in identifying project successes and barriers. This will be sent via email to the project leads in the form of a redcap survey.  
NHSN Surveillance Training To Do:
  • Overdue (minimum of 1 person per facility) Complete the NHSN surveillance training 
  • Sign the Attestation and return it to the Network you will receive the attestation via a Redcap email survey to the facility manager.  (FKC facilities only-NHSN facility completion will be provided to the Network by your corporate management)
Patient Ambassador To Do:
  • Attend the New Infection Checklist Training webinar June 27th  at 1pm, outlining the new observation focus on patient hand and access hygiene and surface disinfection audits (invitations to follow via calls and emails)
  • Continue staff hand washing audits
  • July 1, initiate patient hand and access hygiene, and surface disinfection audits and report findings to the facility manager or  QAPI meeting. 
  • Continue to update the "Days Since the Last BSI " poster. Celebrate "0" infections and brainstorm reasons to eliminate BSIs in your facility
  • Discuss your feelings on the patient ambassador program with your facility manager identifying changes and improvements you would like to see.  Discuss how we can get more  patients involved with their care.
