Dear Project Participants,
IPRO End Stage Renal Disease (ESRD) Network of the Ohio River Valley supports the national initiative to increase the number of patients dialyzing at home. To accomplish this goal  the Network will work with 30% of the facilities in the Network service area to demonstrate a 10% increase in the number of patients that start home training based on data available in October 2018. Each facility in this QIA has received information on the activities they are supposed to undertake to reach this goal. There are no patient exclusions from this program.

The Network has begun a small test of change identifying best practices from the free standing Home facilities; once compiled, the list will be shared across the network service area.    
  
Facility Project Lead To Do's:
  • Overdue:  Complete the monthly Tracker( non LDOs only) identifying seven step patient placement information and identifying barriers and challenges sent to you via a Network e-mail.
  • Participate in a Home modalities poster contest, invitation and requirements will be sent via email to the project lead.
  • Print educational materials for your Patient Ambassadors to distribute, see below.
  • Provide a facility notebook or laptop to share the Home Dialysis Central website for interested patients.
  • Review  the National June National LAN call slides featuring Dr. Lockridge.
  •  Review progress with your Patient Ambassador and invite PA to report at an upcoming QAPI meeting.
  • Assist patient ambassador in providing interested patients with Home program contact information.
  • Increase Facility Awareness: Contact a successful home program in your area and ask them to provide a patient  or staff member to visit the facility and provide their expertise to any interested staff member or patients.  Set up a lobby day, one on one interactions for patients or a lunch and learn to educate staff on home modalities. 
  • Coming soon We are asking for your input with a midpoint project data collection to assist the NW in identifying project successes and barriers. This will be sent via email to the project leads in the form of a redcap survey. 

Patient Ambassador To Do's:
  • Distribute the NCC materials on Home modalities.
  • Direct Interested patients to Home Dialysis Central, a website with unbiased information regarding home dialysis and the NW16 and 18 materials Myth VS Reality of home modalities, see educational resources ( link in left column) 
  • Direct Interested patients to Home Programs in the Area, providing contact information
  • Provide National Kidney Foundation Peer Mentor information
  • Assist with planning a facility awareness activity such as the poster contest or scheduling a visit from a home patient or home program facility staff
  • Discuss your feelings on the patient ambassador program with your facility manager identifying changes and improvements you would like to see.  Discuss how we can get more  patients involved with their care
