New WISHIN Works Video: A Preventable-Readmission Success Story

The newest video in the WISHIN Works series highlights how Community Care, Inc. (CCI), uses the WISHIN Pulse community health record to optimize health outcomes for the patients they serve.

CCI is a managed care organization, which serves the more than 10,000 frail elderly, physically disabled, and intellectually disabled in 14 Wisconsin counties. CCI went live with WISHIN Pulse in February 2017.

The new video documents a case that delivered an almost immediate return on investment, only a few weeks after CCI went live. In the course of managing a patient’s transition from acute care, CCI used information in WISHIN Pulse to prevent an almost certain hospital readmission – a dramatic demonstration of the value of a consolidated, longitudinal community health record containing discharge summaries, medication lists, lab results and other critical clinical data.

Hospital readmissions are costly and payers and providers have substantial financial incentives to prevent them when possible. The Centers for Medicare and Medicaid Services (CMS) estimates that nationally, nearly one in five Medicare patients discharged from a hospital are readmitted within 30 days, at a cost of more than $26 billion each year. For the last several years, Medicare has imposed penalties when preventable readmissions exceed threshold levels established by CMS.
