Important Dates for Employees in 2018

azad administration will continue to send employees advance notifications throughout the year, but to further help you plan ahead, here is a summary of important dates you should keep in mind for 2018:
  • Health Care Open Enrollment: April 1 - Open Enrollment information is distributed to employees as soon as it is available to us from the health insurance companies.
  • 4 01k Open Enrollment / Percentage Contribution Changes: January 1, April 1, July 1, October 1  - Enrollment or percentage contribution change forms need to be sent to azad  administration at least 3 weeks in advance prior the Open Enrollment / Change dates. Please note: Participants may reduce contributions to $0.00 at any time by submitting a change form, but once reduced to $0.00, the participant must wait until the next open enrollment to restart contributions.
  • 401k Investment Election Changes - Investment election changes can be done anytime online through  Fidelity Investments.
401k Open Enrollment

If you are not participating in azad's 401k Plan, hosted by Fidelity Investments, and would like to join, or you need to make changes to your current account, you may do so during the upcoming enrollment period.
 
In order to participate or to make changes to your existing account, the necessary forms will need to be received by this  Friday, December 15th. You may request the required forms by contacting azad administration at  admin@azad.com  or (503) 617-9490.
Holiday Schedule Reminders

As the holiday season is upon us, please kindly inform azad administration of any vacation plans or absences via email at  admin@azad.com. For planned absences, as much advanced notice (preferably 3 weeks), is greatly appreciated. For any unplanned absences, please notify azad administration by phone no later than 8:15 A.M. We are available to take your call anytime during the week starting at 7:30 A.M.
If you will be out of town during the holiday week, please submit your azad  e-timesheet the week before, as well as notify azad administration of your schedule change as soon as possible (if you have not already done so).
