Important Announcement
Every year we update your contact information to be sure we are able to let you know about our events. At that time we also check in to see who still agrees to have their photos of our activities posted on Facebook, the newsletter, or our website. It's also very important that we have emergency contact information, so we can reach someone in case of emergency, and that we are aware of any special medical concerns, such as seizures and allergies.
If you haven't yet turned in our Emergency Contact Waiver and Photo Release form, please CLICK HERE, fill it out, and bring it to the next meeting. If you have a guardian they will need to sign it. Thanks for your cooperation. Sincerely,
Elaine Sinni Plan It Network, Inc. |
