Resource Stewardship Initiative metrics improve
 
The Physician Alliance has been working with practices to improve health care decision making when using medical services, procedures and tests that have been identified as areas of over-utilization. Through Blue Cross Blue Shield of Michigan's Resource Stewardship Initiative, TPA identified three focus areas to improve based on current physician scores and TPA's ranking among other physician organizations in these specific areas. 
 
Over the past year, TPA created quality metric reports, patient education materials and tip sheets related to the focus areas. These resources were shared with physicians and practice staff to encourage conversations with patients and improved care. And through this teamwork,  our metrics decreased!  
 
Decrease Pap smears in women under 21:
1.6% (May 2017) down to 1.5% (June 2018) 
 
Decrease low back pain imaging:
33.38% (May 2017) down to 31.9% (June 2018) 
 
Decrease antibiotic use in adults with bronchitis:
74% (May 2017) down to 71.8% (June 2018)
 
Congratulations to the physicians and practice staff who are working hard to improve these metrics!
Closing gaps improves patient care and quality metrics
 
Identifying and closing gaps in care is an important step in improving quality measures, revenue and patient quality of care. Focusing on closing gaps early can help avoid large, last-minute, time-consuming outreach efforts at the end of the year. 

Tips for creating a successful gaps outreach plan:
  • Launch your outreach early to avoid end of year rush
  • Generate gaps in care reports through your electronic medical record system or TPA's disease registry, Wellcentive
  • Run the report using metrics on your tracking tool (provided by your practice resource team (PRT) member)
  • Send reminder letters to patients showing screenings and tests that are due
  • Follow up with patients via phone calls, emails and/or text messages (based on patient preference)
  • Utilize patient portal for additional reminders
  • Place a copy of worked reports and lists in your patient centered medical home binder, along with the tracking tool to review during PRT visit
The Physician Alliance created a letter template that can be customized by practices to address a patient's specific, needed screening. A variety of patient education materials and posters are available on TPA's website, focusing on breast cancer screening, diabetes, antibiotic use and more.

Diabetes Care Begins with Prevention Webinar

  • Describe the trends in type 2 diabetes and implications for clinical practice
  • Review the evidence that supports referring patients with prediabetes to a lifestyle change program
