The American College of Physicians created multiple helpful brochures to help stimulate and guide conversations with patients on palliative care and end of life decision-making. These resources include advanced care planning tools and information for caregivers. View the available brochures here.

Provider Delivered Care Management webinar helps with billing and coding opportunities
Join The Physician Alliance and Blue Cross Blue Shield of Michigan (BCBSM) to gain valuable information on accurate billing and coding of the 13
provider delivered care management codes. Implementing care managers in practices allows for a higher level of care coordination along with opportunities to improve performance metrics. The BCBSM PDCM program assists in supporting care management. Practices can submit care management codes for reimbursement to BCBSM.
