Care management pilot launched by UAW

The United Auto Workers Retiree Benefits Trust established a partnership with Conifer Health Solutions to pilot a care management project. This project targets Blue Cross Blue Shield of Michigan non-Medicare Trust members who are age 26 or older in Michigan, Ohio and Indiana who are facing illness or medical-related issues. The program is free to patients.

Many Trust members have chronic and complex diseases, along with gaps in care and preventive services. This population may include some patients of The Physician Alliance's primary care physicians. Conifer Health will not interfere with any existing care management services established between physicians and patients. This program will offer support through a nurse care manager, who will work with the patient's provider to create a collaborative partnership to improve the patient's overall health and outcomes.
