It's time for all School Readiness providers to begin submitting their 2017-2018 School Readiness certification paperwork.
Please login to the Provider Portal and select "2017-2018 SR Contracts" and begin completing. The contract is in an editable PDF format that is easy to use.
Please note that your contract will never be submitted incomplete. If you leave out information in any section of the contract, a message box will pop up letting you know what still needs to be completed.
Once completed, you can sign your contract electronically and submit it to the Coalition. You will also need to submit the following mandatory forms:
- Open Notification Form for the 2017-2018 Holiday Schedule
- Excessive Holiday Form School Readiness 2017-2018 Program Year
You can also upload supporting documents such as Certificates of Insurance, Workers Comp certificates, Delegation of Authority forms, etc. These documents will be stored in your Document Library within the Provider Portal to access when you need them.
