How can the design of our hospital demonstrate the inclusion of family & caregivers?

The following themes came out of community engagement conducted in 2015 and reflect what people told us matters most in their health care experience:

  1. Quality experience
  2. The inclusion of family & caregivers
  3. Patient advocacy
  4. Health system navigation
  5. Respect

We invite you to tell us how our space could demonstrate these themes by visiting the Our Healthy Future website.

Growing and changing to provide the care our community needs
The Our Healthy Future (OHF) initiative is a redevelopment planning process  that began in
spring 2015. As of June 2016 we developed a vision for our
future model of care and the facilities we’ll need to deliver that care. The vision was approved by the HHS Board of Directors and was then submitted to the Hamilton Niagara Haldimand Brant Local Health Integration Network and the Ministry of Health and Long-Term Care for review. We are working throughout 2017 to complete and receive approval from the LHIN and Ministry of Health for each part of our Stage 1 submission.
