Reminder from the School Nurse
In order to provide health care in an efficient and safe manner, our School Nurse, Mrs. Long, needs up to date health forms for each student. The Pine Lake Prep web site has a list of health forms here:
http://www.pinelakeprep.org/parents_forms.html
Kindergarten: NC state law requires your student have written documentation of immunizations and Kindergarten Health Assessment from your child's physician by the first day of school. Please visit
immunize.nc.org for the schedule of immunizations that are required.
Seventh Grade: NC state law requires written documentation of Tdap booster and MCV (meningococcal) vaccines by the first day of school.
Students with Health Concerns (Asthma, Food, Ant, Bee Allergy, Seizures, etc.): Students are required to have an Emergency Action Plan on file completed by the parent and physician. Emergency medications should be provided to the Lower School the by
Monday, August 24. Middle and Upper school students should have medication turned into the school or a Self Carry form on file. Students with this form on file should have their emergency medication with them at all times. Only emergency inhalers, epinephrine injectors, and diabetic medications are allowed by law to be carried.
- Please contact Rebecca Long, PLP School Nurse, with any questions at [email protected].
