RFP for REACH Demonstration Grant

The Chicago Department of Public Health's Request for Proposal implementing REACH in the City of Chicago under RFP # DA-41-000J-07-2012-001 is Tuesday, July 31, 2012 at 5:00 p.m. CST. REACH is an initiative focused on decreasing obesity and hypertension health disparities.

The goal of project is to increase: 1) smoke free environments; 2) availability and access to healthy foods, and 3) access to safe places to be physically active for Chicago residents. A total of $400,000.00 to $750,000.00 may be available for funding applications in the City of Chicago. Two Delegate Agencies will be selected to the implement REACH interventions during Nov. 30, 2012 through Sept. 29, 2015.
