Dear Parents,

We hope you are enjoying your summer! The following information will help you prepare as we count down the days to Registration and the start of school.

Preregistration Paperwork
Please complete each of the following BEFORE Registration to expedite the process.
  1. Lunch Benefit Application: Payments such as Student Fees and school lunch payments are based on these results, so it is very important that you complete this application. If you qualified for free or reduced lunch last year, you still need to fill it out to re-qualify for the 2014-15 school year. If you choose not to fill out the Lunch Benefit Application, please contact Pam Burrell via email at [email protected]. The online application is available here.
  2. Student Fees: Student Fee information can be found here. Please take time to carefully assess your fee total; this will help to expedite your time spent in line. Payment for Student Fees is expected at Registration. Payments can be made by cash, check or credit card. If payment plans are needed or if you have any questions, please contact Angie Stattman at [email protected]. All accounts must be settled or arrangements made no later than the first day of school.
  3. Registration Packet: Please find your Registration Packet here. The first four of the following documents are mandatory for the start of school and must be returned at Registration:
    1. Signature page (one per family), which pertains to the 2014-2015 Parent-Student Handbook
    2. Annual Health Update (one per child)
    3. Internet and Flash Drive Policy (one per child)
    4. Physical Education Form, if completed (one per child)
    5. After School Study Hall/ASSH (one per family) and Parking Permit (one per student) can be returned at Registration if applicable.
  4. Health Plans: Health Plans are needed for students with a medical condition, such as, but not limited to, asthma, diabetes and/or food allergies. Any student in need of a Health Plan must have documentation filled out before Registration so the plan is in place by the first day of school. All health forms, including Authorization to Administer Medication (if needed) can be found here. Please direct any questions to Karen Triplett at [email protected].
  5. The 2014-2015 Parent-Student Handbook is available here. Hard copies of the Handbook are available upon request. All parents and students must read the Handbook before coming to Registration. Please sign and return Signature Page once you and your child(ren) have read the Handbook (parent and student signatures required).
