Working Together for Healthy Communities: Models for NM
A number of our New Mexico communities are forging collaborative relationships to create comprehensive and integrated systems of care. The goal is to ensure that all residents, regardless of income, have access to high quality of health care and to all the resources they need for health and well-being. The "double bottom line" encompasses both
better health outcomes
lower health costs.
SHARE New Mexico appreciates the opportunity to partner with two of these initiatives. The Bernalillo County Accountable Health Community and the
Santa Fe County Accountable Health Community
will be presenting on their first year experiences at the
NM Public Health Association Conference
