SVCHC brings outreach services to the homeless

SVCHC has partnered with Sonoma Overnight Services (SOS) to provide outreach services to the homeless population in Sonoma Valley. Since May 2016, SVCHC has hosted two enrollment events at SOS to help enroll homeless patients into care. At both enrollment events, information was provided on our transportation service, health insurance, Cal-Fresh, and numerous appointments were made. 

SVCHC is looking forward to extending more services to the homeless population in the future, including dental and medical care.

Community Teaching Garden

SVCHC has a new community teaching garden on site!  We would like to thank the following donors for their generosity and support in making this project possible:

Teens Standing Together

The second session of SVCHC’s teen group for girls began on May 25 and will continue through July 13.  The group is designed for girls ages 13-17 and open to anyone fitting this category, whether or not they are SVCHC patients. The teen group is run by AmeriCorps worker, Maeve Greenberg, and meets weekly to hear from guest speakers and discuss various issues related to adolescent health. Topics include physical and mental health, self esteem, healthy relationships, and women’s health. In addition to hearing from guest speakers and participating in group discussions, the girls partake in activities such as hip hop, movie nights and art evenings. A big THANK YOU to WestAmerica Bank who donated $1,000 to support our Community Health Fair in October !

The 2nd annual Sonoma Valley Community Health Fair will take place in October of 2016. The goal of the Community Health Fair is to provide health access, education, and services for the underserved community in Sonoma Valley.




  Community Foundation Sonoma County helps fund our Transportation program! 

THANK YOU to the Community Foundation of Sonoma County for supporting our Transportation program! In the 2016-17 year, SVCHC will now be helping provide transportation to/from specialty doctor appointments in outling cities. 
SVCHC receives grant from City of Sonoma! 
SVCHC is a proud recipient of the City of Sonoma grant to help fund our community engagement activities. Funding will be used to support our transportation program and ensuring that all individuals in Sonoma have equal access to care.   THANK YOU City of Sonoma for your support! 

Kivelstadt Cellars hosts Spring Release party to benefit SVCHC
Kivelstadt Cellars hosted their Spring Release Party at the Kivelstadt Vineyard to benefit SVCHC. Attendees enjoyed games, food, and of course great wine!

Back to School supply drive! 
The Back to School Health Fair Committee wants children to learn and succeed at school Help kids in Sonoma Valley succeed by donating school supplies at any of our 2 drop off locations. 

Drop off supplies at SVCHC or at Staples on Napa Road. 

Items needed include:
  • # Pencils
  • Erasers
  • Colored Pencils
  • Notebooks
  • Backpacks
  • 2-pocket folders
  • Paper

Back to School Drive goes through August 8th! 


Walk with SVCHC during 4th of July !

Sonoma Valley Community Health Center will be participating in the parade this year, and  we are seeking volunteers to represent SVCHC in the parade. Be part of our float! Invite your family and friends.

For the past 20 years, SVCHC has addressed the needs of the uninsured and underinsured, low income, and other community members. Each patient seen at SVCHC is one more healthy community member and may be one less uninsured patient seen at the emergency room. 

Your tax-deductible donations help fund a variety of medical services, medications, lab screenings, and supplies in our clinic .
