Identify the key strategic and practical decision points for organizations considering home health and hospice development and growth, and explore a self-study tool to assess an organization's readiness for successful home health and hospice adoption will be featured during LeadingAge's Session 16-A on Sunday, October 28th at 3:30 p.m . David Trost , CEO, St. John's Lutheran Ministries, and Jennifer Schwalm , President, Ashlar Creative Solutions, Elizabethtown, PA and Chief Mission Development Officer, Masonic Villages, will share their insight during a panel discussion with Nancy King , President, Senior Options.
Operationalizing Home Health

Beth Bossert , Chief Clinical Officer, Masonic Villages, Elizabethtown, PA and
Sally Huston , Senior Director of Clinical Operations, Senior Options, Virginia Beach, VA are co-presenters for session 97-E, "Operationalizing Home Health" during LeadingAge. The session will cover understanding how to assess current delivery of care management services across an organization to identify service gaps; learn how to launch and integrate a home health program into an existing care delivery system; discover the qualitative and quantitative measurements being used to determine the success and impact of home health services and overall care management. The session is scheduled for Tuesday, October 30th at 8:30 a.m .
