Even previous patients show immense interest in the new ProLibra - testicular implants and contact our office regarding implant exchange surgery. The main reason for such an interest seems to be the advanced ultra-soft implant material which provide a more natural appearance and feel to the enhanced scrotum. In fact, some of our previous patients report a feeling of �hardened� implants which may occur over time when scar and fibrotic tissue cover the smooth implant surface. The new ProLibra- testicular implants consist of a much softer silicone material when compared to our previous model, which perfectly compensates for the aforementioned issues.

This is what our patients usually report about Pro-Libra: �Hello Dr. Elist,
it's been four weeks since I got the new implants. So far, there were no complications at all. The scarf has healed, doesn't give me any pain or discomfort except that it feels a bit numb. The new implants feel much better and softer and more natural than the old ones.�

The testicular enlargement procedure takes about 45 minutes and our patients see the results immediately after surgery. The end results of the procedure are then appreciated a few weeks after the procedure day when all swelling has resolved showcasing the new testicular shape and size. Testicular implant exchange surgery may take between 60 to 90 minutes, when the old implants are removed and the new Pro-Libra implants are placed inside the scrotum. During recovery, some additional swelling may be present which usually resolve quickly (over few weeks) and the end results and the new feel is appreciated by our patients then.

Penile girth enlargement using autologous fat tissue transfer is one of the oldest and widely used methods for penis circumference enhancement. While readily available from the donor site of the patient, fat tissue might be used to fill out any tissue defects which have resulted from e.g. accidents and injury. The filler function and the aesthetic results of the transferred fat are best observed in areas where preexisting fat tissue makes the survival of the newly transferred fat tissue possible.

The success of a fat transfer procedure usually depends on:

• The location where the harvested fat is transferred to
• The amount of transferred fat
• Patient�s healing properties
• The surgeon�s expertise

While suitable in areas like the face or other soft tissue regions, fat injections into the penis are not the optimal choice of enhancement procedure. In fact, the American Urology Association has even banned fat transfer into the penis for enlargement purposes: �The American Urological Association (AUA) and the Urology Care Foundation consider subcutaneous fat injection for increasing penile girth to be a procedure which has not been shown to be safe or efficacious.�

Fat tissue injected into the penis does not last long and tends to be absorbed after a short period of time (6-12 moths) making repeated injections necessary. Furthermore, fat tissue injected into the penis tends to accumulate in the form of fat islets, is being encapsulated by fibrous tissue, and tends to form nodules underneath/within the penile skin. This is not only dissatisfactory regarding the end results, it also causes penile disfigurement, and in many cases severe skin scarring with associated necrosis.

Patient with previous penile fat injections are usually not suited for other enhancement procedures (e.g. subcutaneous soft silicone implant) without a removal of the exciting fat tissue and extensive skin recovery.
