Exemption TC-40D with your TAXES?

Is your child in Early Intervention services or on an IEP (Individualized Education Plan)?  If so then you may be eligible for the TC-40D exemption form.  Visit our website to download your form and see if your child is eligible.  Do not submit the TC-40D form with your return. Keep a copy of the form with the authorized certification signature filled out by the appropriate state agency in your records in case of an audit.

Looking for a support group in your area? W e have you covered with our Family to Family Network. They can help you by providing:
  • support through local monthly meetings, either in-person or online
  • information and referral
  • family to family support
  • advocacy for families
  • help to families who qualify or may be eligible for services from the Utah State Division of Services for People with Disabilities (DSPD)  
  • Help them understand programs and services and how to work with service providers and support coordinators

UPC Welcomes Paige Albrecht to our team!

The Utah Division of Services for People with Disabilities (DSPD) is administering a survey to find out what parents or guardians of teens with a disability think about post-high school opportunities for their student, specifically barriers and potential to pursue higher education and employment.  Ultimately, the researchers hope to use the findings to help improve post-high outcomes for people with disabilities.

The survey should take between 10-15 minutes to complete.  Thank you for your participation.  Click here to begin survey.

Post-Secondary Decisions? 
 
What are your plans after high school? What postsecondary education options are available for individuals with disabilities? Is college right for me?

Post-secondary education is not simply an extension of high school. Students who decide to continue their education beyond high school will need to choose among vocational programs, two-year colleges, or four-year universities or colleges, in addition to deciding on an educational path. Keep in mind that many programs can be tailored to your child's needs and interests.

Here are some helpful resources to get your student ready for their new journey!
For those of you in Northern Utah, please join an open-house event: BEYOND HIGH SCHOOL.   Thursday, April 13 , at Ridgeline High School in Millville (Cache School District) from  6 to 8 pm.

The event will feature vendors and discussion about postsecondary education opportunities for students with disabilities. Please invite students with disabilities and their family members. Click here for information.

Medically Complex Waiver for Ages 0-18

Applications for the Medically Complex Children's Waiver will be accepted during an open application period  May 1-31, 2017.

Purpose:  This program provides medical assistance to children who are medically complex.  This waiver serves the following population:
  • Children ages 0-18
  • Children who have 3 or more specialty physicians
  • Children who have 3 or more organ systems involved in their disability
  • Children who have an SSI Disability Designation through the SSA or a disability determination by the State Medical Review Board; this will be coordinated as part of the MCCW application process.

The overall goal of this interactive workshop is to help parents identify the correct benefit that applies to their child or transitioning adult! We will cover SSI for children 18 and younger as well as transitioning students 18+.

Behavior Strategies for Teens

Thursday, April 13, 2017 from 6:30 PM to 8:30 PM MDT

The purpose of this workshop is to introduce families to the ways behavior can be addressed through the special education process or through Section 504 services. The workshop gives an overview of how to find the causes of behavior problems and how positive behavior interventions can help at home and in the school.

Related Services- Finding and Funding Them

Thursday, April 20, 2017 from 7:00 PM to 8:30 PM MDT

Related services: speech, occupational therapy, physical therapy and assistive technology - needs don't end when school ends. Learn what resources are available to keep these services intact and some ways to cover the costs.
