The Be Thoro Safety Report Form
Fellow Thorobreds,

The health, safety and well-being of our students and employees is a top priority at Kentucky State University. BREDS Back on the Hill was developed to ensure we repopulate Kentucky State University in the safest manner possible. The health and safety of our entire campus community is a shared responsibility. While we are hopeful that each of you will adhere to the requirements outlined in the guide and that in many cases where violations occur will require a simple and friendly reminder between friends and colleagues, we recognize some matters might need further review.

If you see a violation of the University’s established safety guidelines, please complete the COVID-19 Be Thoro Safety Report Form. Reports may be submitted anonymously. Use this form to report only non-emergency concerns.

Examples of reporting include:
  • A violation of an isolation or quarantine order
  • A violation of the University’s operational alert status
  • A member of the campus community coming to work or class with COVID-19 symptoms
  • Inappropriate mask or face covering usage
  • Removal of COVID-19 signage or safety barriers
  • An unauthorized event or activity
  • An unauthorized visitor(s) on campus
  • Social distancing
  • Sanitation concern

If you have an emergency, please dial 911 or contact University Police at (502) 597-6878.

Be THORO and stay safe.
