Parents of the Class of 2020,

All juniors in the State of Pennsylvania are mandated to submit a physical along with immunizations.
This is NOT interchangeable with the PIAA sports form required for sports.

All seniors are required to get an additional vaccine, the meningococcal conjugate vaccine. This vaccine may be given at the same time of the Junior Physical. This vaccine is required to attend school next year (senior year).

Please submit a record of this vaccine as soon as your child receives the MCV. The deadline is the first day of school.

If you have any questions or concerns please email me:  nurse@jcarroll.org
