This month we highlight SFH ACTION (Accelerating Change Together In Ontario) - a collaborative of 87 hospitals across Ontario that have been engaged in process improvement initiatives which promote senior friendly care.

    The SFH ACTION Collaborative Showcase on March 2-3 boasted top notch speakers from the UK and Canada, including:

    Need a handout to help you depresecribe?
    Dr. Cara Tannenbaum, Director, Canadian Deprescribing Network, Professor of Medicine (Geriatrics) and Pharmacy, Université de Montréal gave a compelling keynote address on deprescribing. She has provided the following handouts which may be  printed and used free of charge for non-profit patient and educational purposes.
