The ORN and KidneyWise Toolkit Have Changed My Practice... I Think They'll Change Yours Too:
2015-16 OCFP President Dr. Sarah Newbery
The Ontario Renal Network's KidneyWise toolkit has changed how OCFP President Dr. Sarah Newbery manages patients with or at risk for chronic kidney disease. In her latest blog, she discusses the toolkit with Dr. Allan Grill, Provincial Primary Care Lead at the ORN and one of the leads for the project.

Training Future Leaders in Refugee Health 
 
The Canadian Collaboration for Immigrants and Refugee Health welcomed 32 first-year medical students from across across Canada, Cairo and Beirut to its fourth National Summer Institute on Refugee Health in June. Participants gained knowledge and tools to develop and lead refugee health outreach programs.

Telehomecare Supports Chronic Disease Management

Ontario Telemedicine Network's
Telehomecare is a self-management program that helps patients with COPD and congestive heart failure who are living in their home. Currently available in 10 Local Health Integration Networks, with plans to expand throughout the entire province, the service is provided at no cost to patients or health-care providers.

Mentoring Networks Help Docs Address Complex Care
In June the OCFP's Collaborative Mental Health Network (CMHN) and the Medical Mentoring for Addictions and Pain Network (MMAP) hosted a regional meeting in Windsor, Ont., and welcomed physicians from the area to discuss caring for patients with complex mental illness, addictions and pain issues.
